What do hematopietic growth factors act on?
-hematopoietic growth factors act on the bone marrow to increase production of RBC
What is Epoetin used for?
-anemia of CRF
-HIV clts taking Retrovir
-anemia induced by chemo
-anemia in clts scheduled for elective surgery
What are some SE for Epoetin?
-HTN 2/2 elevations in HCT
-increased risk for CV event
What are some Nrsg interventions for a clt on Epoetin?
-monitor clts Fe+ levels
-RBC growth dependent on adeqauate quantities of Fe+, folic acid, & vit B12
-monitor clts Hgb & HCT 2x/wk until target range is reached
-obtain baseline BP
-in CRF, control HTN before tx
-do not combine Epoetin c other meds
How would the RN evaluate the effectiveness of Epoetin?
-Hgb levels of 10-12 & HCT of 40%
-increased reticulocyte count
What do filgrastin (Neupogen) & pegfilgrastin (Neulasta) do?
-stimulate the bone marrow to increase production of neutrophils
-decreases the risk of infection in clts c neutropenia
What are some SE of filgrastin (Neupogen) & pegfilgrastin (Neulasta)?
-bone pain
-leukocytosis (decrease dose or stop tx if WBC >50000 or platelets >500000)
-->contraindicated in clts sensitive to E. Coli
-->should not be combined c other meds
What would a RN look for when evaluating filgrastin (Neupogen) & pegfilgrastin (Neulasta) effectiveness?
-absence of infection
-in chemo for CA tx, an absolute neutrophil count increase to >10000 after chemo induced nadir
What does sargramostim (leukine) do?
-acts on the bone marrow to increase production of WBC (neutrophils, monocytes, macrophages, eosinophils)
What is sargramostim (leukine) indicated for?
-facilitates recovery of bone marrow after bone marrow transplant
-used in tx of failed bone marrow transplant
What are the SE of sargramostim (leukine)?
-diarrhea
-weakness
-rash
-bone pain
-leukocytosis
-thrombocytosis
-->reduce tx if WBC >50000; neutrophils >20000; platelets >500000
-->contraindicated in clt allergic to yeast products; use cautiously in clts c heart disease, hypoxia, peripheral edema, pleural & pericardial effusion
How would the RN evaulate the effectiveness of sargramostim (leukine)?
-clt has absence of infection
-WBC & differential c/in normal ranges
