What considerations should be made before the 1st assessment of a patient? |
- have post-op instructions been read? - did the patient have a nerve block? - does pt have any relevant PMH or current medical conditions that may impact participation in therapy? - is the patient medically able to participate? - has the patient had analgesia recently? - does the pt have any anxiety, cognitive, confidence, communication problems? |
What sort of PMH or PC conditions may affect a persons ability to participate in therapy? |
Parkinson’s Disease, CVA, previous arthroplasties, fractures |
What factors are considered to confirm whether a patient is medically fit to participate in therapy? |
NEWS readings, leaking wounds, bleeding, any investigations |
What should you ensure is prepared with the patient before assessing? |
- is the patient wearing glasses/hearing aid - is the patient alert/orientated - is the environment clear and safe? - has a suitable walking aid been chosen? - does the patient have suitable footwear to mobilise in? |
What aspects could be considered when preparing the environment for assessing a patient? |
- dry floor -no obstacles - wires, cables, clutter, shoes - catheter, IV, PCA lines are organised to be able to move with pt |
What are potential risk factors / post-op complications |
- Unstable obs - post-op confusion - excessive pain -excessive wound leaking/bleeding, other fractures - DVT - decreased sensation / compartment syndrome - abnormal lower limb weakness, change in tone - hip dislocation |
What are the 4 main physical factors that should be assessed on a patient? |
- vision, hearing and cognition - sensation - upper limb function - lower limb function |
Why should you assess vision, hearing and cognition? |
To ensure pt has sufficient sensory and cognitive abilities to take part in therapy |
Why should sensation be assessed? |
To determine suitability for mobilising the patient |
Why should upper limb function be assessed on a hip replacement patient? |
To ensure pt has enough strength and function to complete in bed transfers and use the appropriate walking frame |
Why should lower limb function be assessed? |
To assess if patient can transfer to the edge of the bed, move feet to the floor, stand to the mobility aid and then transfer/mobilise providing that muscle strength and control is sufficient |
What 3 main pieces of information should you check in a patients files before assessing them for mobilising? |
- operation notes / post-op instructions - patients history of presenting condition - checking NEWS values |
