What is the difference between anatomic and extra anatomic graft? |
Anatomic follows the course of normal anatomy. Extra anatomic this placed where a vessel typically does not run. |
Type of graph materials*** |
Synthetic or autogenous Examples of synthetic polytetrafluoroethylene or Dacron Example of autogenous, patients own artery or vein |
What are the two types of autogenous vein graphs? |
In- situ Reversed |
Describe in-situ vein graft. |
Vain is left in place and proximal and distal and is connected to artery bypassing occlusion. And the valves are destroyed to allow for flow. |
Describe reversed vein grafts. |
The vein is removed and reinserted reversed to allow for flow so that the one-way valves do not have to be removed |
What is the indication for arterial duplex evaluation following intervention? |
Routine post-op surveillance or any other symptoms suggesting graft failure |
What is an reverse vein graft risk? |
The retained valves tend to be common sites for stenosis. |
What is a common risk with in-situ vein grafts? |
Any tributaries that were not ligated may become arterial venous fistulas |
What is an anastomosis? |
The area where the newly connected vessels connect |
What parts of a graft need to be evaluated? |
The inflow artery, proximal anastomosis, body of the graft, distal anastomosis, outflow artery, and any observed stenosis |
Interpretation of bypass grafts ***$$$@ |
Define angioplasty |
Surgical repair of a vessel |
How does a stent work? |
A stent is inserted through a catheter, into the stenosis, the stenosis is then opened up by a balloon, the stent is then deployed, expanded, and left there to keep the vessel open. |
What pattern of velocities is common in stents?***$$$@ |
The velocity will usually be uniformly higher throughout the stent |
The 2:1 ratio rule can also be applied to?*"* |
Stents and grafts |
Treatments for aneurysm repair |
Open surgery w insertion of synthetic bypass graft Endovascular aneurysm repair EVAR |
Describe an endovascular aneurysm repair |
A graft is placed inside of the aneurysm through a catheter and hooked into place with stents. The aneurysm is not replaced but simply excluded from the main artery |
What is an endo leak in a endovascular aneurysm repair?$$ |
Flow within the aneurysm sac outside of the graft walls |
After an endovascular aneurysm repair what should be measured over time?*** |
The diameter of the aneurysm sack should be measured as it should decrease over time. And enlarging sack is an indicator of a potential endoleak |
Types of endo leaks***$$$ |
Intravascular ultrasound IVUS |
