-coarse tremor, GI upset, Ataxia, Dysarthria, Seizure, Confusion.
What are the major nursing interventions for Lithium Carbonate (Eskalith)?
*VERY NARROW THERAPEUTIC WINDOW (0.5 - 1.5)*
-draw blood 12 hours before each dose.
-Keep salt levels the same
-Low salt = high lithium = toxicity.
-Don't take with diuretics or NSAIDs
What is the indication for Nardil?
Atypical depression - Depression resistant to other meds.
What are the side effects of Nardil?
Postural Hypotension, Insomnia, Sexual Dysfunction, Weight Gain.
What are the major nursing interventions for Nardil?
-Must eat a low tyramine diet (avoid any aged foods such as cheese or wine.)
-See MD ASAP if any hypertension occurs.
-Watch for suicide tendencies
What are the indications for Elavil?
Depression, Anxiety.
What are the side effects of Elavil?
Sedation, Weight Gain, Anticholinergic (think atropine)

-Suicide assessment! - 3 weeks supply is toxic! (deadly OD)
-No driving
-Stay on meds >1 year

Positive symptom psychosis.
What is a positive symptom of pyschosis?
Hallucinations, Delusions. Something they have and we do not.
What are the side effects for Thorazine and Haldol?
-EPS: Pseudoparkinson's, Acute Dystonia Reaction, Akathisia, Tardive Dyskinesia
-Neuroleptic Malignant Syndrome
-Hypertension
-Sedation
-Lowers seizure threshold
What are the major nursing interventions for Thorazine and Haldol?
-Educate pt about EPS
-Medicate EPS symptoms PRN
-Reduce dosage or give anticholinergic if necessary
What is the indication for Clozaril and Zyprexa?
Negative symptom psychosis
What is a negative symptom of pyschosis?
Lack of motivation or emotion; apathy.
What are the side effects of Clozaril or Zyprexa?
Sedation, Drooling*, Hypotension, Significant Weight Gain*, Few EPS, Agranulocytosis with Clozaril.
What are the major nursing interventions of Clozaril or Zyprexa?
-Educate about weight gain - could lead to Type 2 Diabetes
-Check WBCs weekly
-Check national registry for agranulocytosis with Clozaril
-Drooling may be forever.
What is agranulocytosis?
Degranulation of WBCs - which can lead to bleeding out and infections easily.
