HMG-CoA reductase inhibitors-"statins" awesome but terrible AE

Bile acid sequestrants-Bile has lots of lipids; uses bile to control cholesterol

Niacin-Brings down cholesterol

Fibric acid derivatives-absorb cholesterol

Cholesteral absorption inhibitor (Zetia)
HMG-CoA Reductase Inhibitors "Statins"
*Inhibit* HMG-CoA enzyme which decrease cholesterol production (lowering LDL)

Drug: atorvastatin/Lipitor (P)

SE: GI disturbance, increase liver enzymes

NrsConsiders: Must do liver lab, take @ night, drug can last up to one month
Bile Acid Sequestrants
Gathers cholesterol and does not release it. (Decreases cholesterol absorption)

Drug: cholestyramine/Questran
Fibric Acid Derivatives
Decrease synthesis of VLDL

Drug: gemfibrozil & clofibrate

Generally well tolerated, GI complaints most common
Nicotinic acid/Niacin/Vitamin B3
Effective treating most hyperlipidemias but needs Major Dose and must take w/ Antihistamine

Various Effects @ different doses

low tolerance due to flushing, itching, peptic ulcer, diabetic intolerance
Nursing Considerations
-Monitor liver and renal functions
-Monitor cholesterol levels
-Give meds w/ evening meal
-DO NOT CRUSH TABLETS
-Adminsiter w/ 120-180mL of water
-Eat raw vegies, fruit & bran, low fat diet
-Moderate Exercise
-Drink at least 2L water each day
