HMG-CoA Reductase Inhibitors:

-STATINS
Lower LDL
blocks 1st step in cholesterol synthesis; incr LDL receptors on hepatocyts; decr VLDL

Elevate liver enzymes (ALT); myopathy and rhabdomyolysis

CONTRAINDICATED IN PREGNANCY
FIBRATES:

CloFIBRate
FenoFIBRate
GemFIBRozil
Lower serum TRIGLYCERIDES, LDL; incr HDL

Type III (Dysbetalipoproteinemia), and IV & V

PPARa

Myopathy
GI symptoms
Cholelithiasis
Compete for Hepatic Enzymes

Well absorbed when taken with food. Highly protein bound. Will displace warfarin and other highly protein bound drugs when taken concomitantly
Gemfibrozil inhibits hepatic uptake of HMG-CoA Reductase Inhibitors

CONTRAINDICATED IN PREGNANCY AND LIVER DISEASE
NIACIN VIT B3
Significantly increases HDL. lowers rest

Types II-V

Flushing (prostaglandin mediated)
Pruritis (itching)
Impaired Glucose Metabolism (contraindicated in Diabetes Mellitus)
Hyperuricemia and gout
GI side effects
Hepatotoxicity
Avoid use in Pregnancy
BILE ACID SEQUESTRANTS:

COLEstipol
CHOLEstyramine
COLEsevelam
Inhibit reabsorption of bile salts.
Induce the conversion of cholesterol to bile salts rather than VLDL production
Induces synthesis of LDL receptors and removal of LDL from circulation

GI symptoms- constipation, flatulence, nausea

Impaired absorption of vit. A,D,E,K

Impaired absorption of Warfarin, Tetracycline, Digoxin, Phenobarbital, Statins
EZETIMIBE
Cholesterol absorption inhibitor

Mild decrease in cholesterol, LDL, triglycerides

Undergoes phase II conjugation
Avoid in patients with hepatic dysfunction
