What is Clomipranine used for?
Only OCD
Monoamine oxidase inhibitors
Phenelzine
Tranylcypromine
what are the 3 diff types of antidep?
Tricylcics
Monoamine Oxidase inhibitor
Selective Serotonin reuptake inhibitors
Tricyclines blocking acetyl and histamine is that theraputic?
not therauptic effect
Postural hypotension
fainting upon standing up
Tricyclics side effects?
all drugs haev same side effects drowsiness, blurred vision,
Uses of Tricyclics
Very effective in chronic neuropathic pain
What is Imipranine used for?
(antidepressant)
Used for ADHD
What is the bad thing about antidepressants?
One of the ways depressed patients attempt suicid is ODing on antidep pill- looks like- cardiac arythmia, cardiac depression, excitiment, delirium, convulsions, respiratory depression, coma
Are tricyclics still used for depression?
no they are too dangerous
What does monoamine oxidase inhibit the break down of?
Inhibits break down of dopamin, norepinephrine and seratonin
Monoamine Oxidase Inhibitors irreversibly do what?
Irreversibly prevent breakdown of monamines inlcuding: dopamin, norepinephrine and seratonin
What does the moi do to the body?
It will cause hypothermia and need for hospitalization takes two weeks to get maois back
If taking monoamine oxidase inhibitors what foods should u stay away from?
cant take foods with tyromine, cheese, wine, chocolate- because it builds up more norepinephrine
Selective serotonin reuptake inhibitors (SSRI's) (antidepressant drugs)
Given alot now for depression
OD of SSRI, what happens?
Serotonin syndrome- invloves- confusion, agitation, fever, hypertension, increased reflexes, halucinations
no need for long hospitalization
Side effects of MAOIs
weight gain, sedation, orthostatichypotension= posturohypotension
Side effects of SSRIs
insomnia or sedation
nausea agitation headaches
sexual dysfunction
SSRI's list
celexa, lexapro,prozac,luvox,paxil, zoloft
SSRI withdrawal syndroms- abrupt stop
flu like symtpoms, nausea, vomitting, diarreah, dizziness, parasthesia- numbness and tingling of extremities, vivid dreams, anxiety and crying, depression, memory probs, confusion,
Atypical antidepressants
newer drugs made after ssris
Mirtazapine (remirone) Atypical
alpha 2 antagonist increase levels of norepinephrine
DRI
Dopamine Reuptake Inhibitor
NRI
Norepinephrine reuptake inhibitor
SRI
Non selective serotonin reuptake inhibitor
SNRI
Seratonin norepinephrine reuptake inhibitor
plavix cause and ssris
cause pulmonary hypertension and heart defects in newborns
Pregnancy category D -plavix
Is prozac dangerous?
yes it is black boxed
Anxiety Agents
Benzodiazepine
Nonbenzodiazepine
mechanism of action of benzodiasepine
increases gaba by working in the brain at a gaba complex- bind to benzodiazepine receptors
Do we have endogenous benzodiasepines?
Yes we do, foods also have benzodiasepines
Whats bad about benzodiasepines?
Cause addiction, physical dependence
What doe benzodiasepines do outside of antianxiety?
they are sedative, relax muscles, tension headaches, used in surgurey for so you dont remember surgery
What kind of amnesia do Benzodiasepines cause?
cause anterograde amnesia
lithium and benzodiazepines side effect
look like they are drunk
what class is valium?
It is schedule IV and highly addictive
what is bad about Ativan?
Worst to get off of, withdrawal is extremely bad
all benzodiazepines are which schedule?
Schedule Iv
Are benzo's ok for pregnancy?
No they cause floppy infant syndrome- hypotonia, hypothermia, decreased respiration, hyperexcitable, delayed motor coordination
what are effects of Taking benzo's for long term
starts off with a lot of gaba and inhibiting glutamate, long term
same release of gaba but glutamate increases to compensate for benzodiazepines- symptoms will be opposite benzos
Is it good to get off benzo's?
no, brain will get worse, more glutamate will form each time u go back on- glutamate is toxic to brain
Is glutamate good for the brain?
No it is toxic for the brain
Nonbenzo's
almost all have serotonin reuptake inhibition, do not cause addiction
Benzo names to know
Xanax Valium and Ativan
fear
if enough serotonin in amygdala it will decrease fear
Non benzos cause
dry mouth, dizziness
Hydroxazine is not an antipressant but a?
antihistamine
Desyrel
Causes retrograde ejaculation,
priopism- also ssri's cause this- prolonged erection.
desyrel also causes
altered libido, retrograde ejactulation- semen backs up into urine
What are Sedative Hypnotics
sleeping pills- hardest drug to get off of. benzodiazepines, and nonbenzodiazepines
Side effects of sedative hypnotics
benzos-daytime sedation- depends on half life of drug, nonbenzos- all drugs except for roserem are gaba agonists
Non benzo sedative hypnotics addictive like benzos?
yes except for roserem
Side effects of lunesta (nonbenzos)
memory probs, tolerance, withdrawal, dependence,
Is lunesta easy or hard to get off?
very hard
Behavior changes on lunesta
more aggressive, hallucination, assertive, suicidal thoughts,
Ambien (sedative hypnotic) side effects
sleep driving, eating, having sex, sleep walking, making and eating food,
