Relieved with rest or nitroglycerin.
Unstable Angina:
Occurs with
Occurs with exercise and/or stress.
Increases in occurence, severity and duration.
Variant Angina (Prinzmetal) occurs d/t
coronary artery spasms.
unrelated to exercise or stress.
Myocardial Infarcrion (MI)
rapid progression of myocardial necrosis.
d/t imbalance of O2 supply and myocardial demand.
The difference btwn angina and MI is pain....
Unrelieved by rest or nitro.
Lasting more than 15 mins.
S/s of MI
Chest Pain (ant. pericardium - tight, pressure, or squeezing)
Radiates to jaw, neck, arms, back, and epigastrum.
Dyspnea
Nausea w/o vomitting, abdo pain.
Anxiety
Lightheadness with or w/o syncope
cough
sweating (diaphoresis)
Cardiac Enzymes:

Norm: 0% of total CK; 30-170 units/L.
Elevated levels first detectable: 4-6 hrs

Medication tx:
Nitrates - Reduce preload and symp relief.
SubLingual
Nitroglycerin sublingual stat 0.4 mg q 5 mins until pain relieved or x3 doses.
Medication tx:
Nitrates-
IV
Indicated for reliefe of ongoing ischemic discomfort, control of HTN, or management of pulmonary congestion.
Medication tx:
Morphine Sulfate -> vasodilation, reduces preload and O2 consumption.
2-4 mg IV stat and prn for adequate pain relief.
Medication tx:
Asprin 160 - 325 mg tab
Admin immediately if not already taken at home.
Shown to decrease mortality.
Use Plavix if allergic to aspirn.
Medication tx:
Thrombolytics - disolve clot causing ischemia/infaction.
Administer ASAP after onset of symptoms (w/in first 6 hrs following MI).
Other Medication tx
Beta blockers
ACE Inhibitors
Do not administer Nitrates to clients who have taken ______ for _____ dysfuction with in the last 24 hrs.
phosphodisterase inhibitors for erectile dysfunction
What to do when chest pain occures
stop activity and rest.
Place nitroglycerin table under tongue q 5 mins. If pain is unrelieved after 15 mins, call 911.
