Cranial Nerve I

- Olfactory Nerve (Sensory)


- ask patient to smell something testing each nostril separately (without looking)

Cranial Nerve II

- Optic Nerve (Sensory)


- Test 1: Face patient nose to nose having them look you directly in the eyes. Wiggle your fingers at the edges of your field of vision and ask patient to tell you when they can see motion


- Test 2: Have patient read the smallest line they can see on the chart one eye at a time.

Cranial Nerve III

- Oculomotor Nerve (Motor)


- Test: have patient follow your finger through the 6 cardinal fields of vision and then end with the patient following your finger as you move it towards their nose.

Cranial Nerve IV

- Trochlear Nerve (Motor)


- Test: have patient follow your finger through the 6 cardinal fields of vision and then end with the patient following your finger as you move it towards their nose.

Cranial Nerve V

- Trigeminal Nerve ( Sensory and Motor)


- Test 1: Lightly sweep across the forehead, cheeks, and jaw line.


- Test 2: Have patient clench their jaws and palpate the firmness of the right and left temporalis mm.

Cranial Nerve VI

- Abducens Nerve (Motor)


- Test: have patient follow your finger through the 6 cardinal fields of vision and then end with the patient following your finger as you move it towards their nose.

Cranial Nerve VII

- Facial Nerve (Sensory and Motor)


- Test: Ask patient to move facial muscle: frown, wrinkle your forehead; close your eyes tightly, smile, purse your lips, puff out cheeks.

Cranial Nerve VIII

- Vestibulocochlear Nerve (Sensory)


- Test 1: have patient cover one ear. Stand 1ft away from patient and whisper a word into their ear and have them repeat it. Test both sides.


- Test 2: Have patient stand on one leg with eyes closed. Test bilaterally.

Cranial Nerve IX

- Glossopharyngeal Nerve (Sensory and Motor)


- Test: have patient say "AH"

Cranial Nerve X

- Vagus Nerve (Sensory and Motor)


- Test: have patient say "AH"

Cranial Nerve XI

- Accessory Nerve (Motor)


- Test: Have patient shrug their shoulders then apply resistance. Have patient turn head side to side to test the sternocleidomastoid

Cranial Nerve XII

- Hypoglossal Nerve (Motor)


- Test: Have patient stick out their tongue and move it side to side.
