Thyroid Gland
-Located at
vertebral levels C5-T1
Laterally the thyroid gland is in contact with
the carotid sheath
Right and Left lobes connected by
the isthmus
Thyroid Gland Vasculature
Superior Thyroid Artery
-Inferior Thryroid Artery
Thyroidea ima artery
The superior and middle thyroid veins
Superior Thyroid Artery: It is a branch of the
external carotid artery. It pierces the superior end of the thyroid gland
Inferior Thryroid Artery: It’s a branch of the
thyrocervical trunk
When present, the thyroidea ima artery enters the thyroid gland from
inferiorly, near the midline
The superior and middle thyroid veins are tributary to the
internal jugular vein.
The right and left inferior thyroid veins drain into
the right and left brachiocephalic veins, respectively
Posterior to the thyroid lobe, in the groove between the trachea and esophagus, we find the recurrent
the recurrent laryngeal nerves
what to do To maintain proper serum calcium levels after thyrodectomy?
at least one parathyroid gland must be retained during surgery
Root of the neck is the junction between
the thorax and the neck, just superior to the superior thoracic aperture.
The only tributary to subclavian vein.
External jugular vein
Subclavian vein- begins at lateral margin of rib 1 as continuation of
axillary veins
Thoracic duct – joins the venous system at
junction of left subclavian vein and left internal jugular vein (Left venous angle)
Right lymphatic duct – joins venous system at
Right venous angle
Right recurrent laryngeal nerve branches off right vagus nerve as it passes anterior to the
subclavian artery
right recurrent laryngeal nerve travels along
along lateral surface of trachea and esophagus up to larynx
Left recurrent laryngeal nerve – branches off left vagus nerve as it passes
the left side of the aortic arch. Travels along lateral surface of trachea and esophagus up to larynx
phrenic nerve passes anterior to
anterior scalene muscle
inferior cervical sympathetic ganglion is located at
at root of the neck
Cervical sympathetic trunk is continuous with
thoracic sympathetic trunk. Will be studied in depth later!
Upper trunk
Middle trunk
Lower trunk composed of
C5-C6.
C7-C8
C8-T1
Interscalene triangle - Formed by
borders of rib 1, anterior scalene and middle scalene (which both attach to rib 1).
- Subclavian vein, suprascapular artery, transverse cervical artery pass anterior to
anterior scalene m.
Axillary a. 1st part
from origin to medial border of anterior scalene muscle. Has 3 branches:
Vertebral artery
Internal thoracic artery
Thyrocervical trunk
Suprascapular artery
Transverse cervical artery
Internal thoracic artery – supplies
ant. thoracic wall, arises from anteroinferior surface of subclavian a.
Suprascapular artery - passes laterally and posteriorly, supplies
supraspinatus/infraspinatus mm. Lowest branch of these 3
Transverse cervical artery – passes laterally, crosses deep to the omohyoid m and superior to the clavicle,
supplies trapezius
Inferior thyroid artery – passes medially toward thyroid glands posterior to the cervical sympathetic trunk. Supplies
thyroid gland. Has 1 branch:
Ascending cervical artery – supplies prevertebral mm. and gives branches to supply spinal cord
2 part of axillar aa
Costocervical trunk
1-Deep cervical artery
2
deep cervical l artery
ascends in back of neck, anastomoses with descending branch of occipital artery, gives branches to supply spinal cord
Supreme intercostal artery – gives rise to
posterior intercostal aa. 1 and 2
3 part of axillary
: lateral border of anterior scalene muscle to lateral border of the rib 1. Has 1 branch.
Dorsal scapular artery – passes between superior and middle trunks of brachial plexus, supplies muscles of scapular region. (note: may arise from transverse cervical 30% of the time, but not in our body)
