Class

Antidysrhythmic agent

Mech

Contains multiple antidysrhythmic properties and is a Na channel blocker, K channel blocker, Ca channel blocker, B blocker and an A blocker. Mainly classified as K blocker.


Lengthens phase 3 of cardiac myocyte action potential increasing the refractory period of atrial, ventricular, AV nodal and Purkinje cell.


Decreased automaticity of the SA mode. Decreased conduction in the AV node and prolongation of the QT interval

Pharmacokinetics

Rapidly absorbed after IV administration. Long duration of action, immediate maintenance drip usually not required

Indications

V fib/pulseless V tach. Wide complex regular tach with pulse including VT and WCT

Contra

None

Side effects

Hypotension; bradycardia; prolonged QT

Dosage

Cardiac arrest: 300 mg IV bolus if needed repeat 150 mg 3-5 mins. If converted maintenance infusion is 1mg/min



Tach: infusion of 150 mg over 1st ten minutes. May repeat once
