What are some predictable changes with the Aging Heart?
Progressive angle between LVOT and ascending AO
Annular calcification (MV and or AV)
Senile Calcific AS
Increasing myocardial stiffness (greater if accompanying LVH)
Thickening of the Aorta
Mild focal thickening of MV & AV
If accompanied by lonstanding, poorly controlled HTN can mimic genetic HCMPY
Mildly reduced CO (diminished LVOT VTI/peak velocity)
Preserved radial myocardial systolic fxn
What finding may accompany the progressive angle between LVOT and AAO?
Localized prox septal hypertrophy
& may become obstructive- usually jet creates turbulence and possibly murmur
What are the signs of increasing myocardial stiffness?
Diastolic dysfxn
LA dilation
Increased RVSP
Greater prevalence of A-Fib
What are some accompanying findings with mildly reduced CO (diminished LVOT VTI/peak velocity)?
Secondary to decreased longitudinal systolic fxn (descent of base)
Diastolic dysfxn (mitral inflow patterns, RUPV flow patterns, decreased E’velocity )
Why is all this important?
To recognize pathology in younger pts which may be expected in the aging
What are predictors of Cardiac Events Even in the elderly?
LA size
LVH
LV systolic and diastolic FXN
What are some classic aging heart findings?
LVH
LA dilation
Thickened MV/TV
Good Transverse systolic movement, poor descent of base (implied diast. Dysfxn)
