drug interactions: charcoal interferes with function of antidote for Tylenol OD, milk decreases effectiveness


dosage: Adult [1gm/kg], Peds Same. notes: pts must be able to protect airway

Albuterol

class: sympathomimetic, Beta 2 adrenergic bronchodilator indications: treat reversible airway obstruction caused by: wheezing(asthma), COPD(emphysema), chronic Bronchitis


contraindications: hypersensitivity drug interactions: beta adrenergic agents ^effects, MAO inhibitors- hypertension


dosage: Adult: [2.5-5.0 mg] 10mg MAX in 3mL NS Peds: [1.25-2.5 mg] 5mg MAX in 3mL NS, 5-15 mins as needed.


notes: side effects dose related, may have paradoxical effect

contraindications: intracranial bleed, DTs with dehydration, administration through same set as blood, unconscious for oral glucose, suspected CVA

notes: may have immediate effect in dystonic reactions, no early benefit to allergic reactions

glucagon

class: hormone hyperglycemic agent


indications: documented hypoglycemia BGL <60mg/mL when IV/IO can't be started, Beta blocker OD w/serious S&S, anaphylaxis refractory to epi


contraindications: pts unable to receive supplemental glucose, hypersensitivity to pork or beef, use w/caution on pts w/pheochromocytoma


drug interactions: hyperglycemic effects intensified and prolong by epi, will precipitate when mixed with calcium


dosage: Adult[0.5-1mg] IM may repeat 10-20 mins if no response, Peds [0.1mg/kg] IM may repeat 10-20 mins if no response


notes: pt must be given supplemental glucose, glucagon is in powder form and must be reconstituted in NS or sterile water 1mL:1mg

Hydromorphone

indications: analgesia for moderate to severe pain


contraindications: hypersensitivity, HTN, head/ABD injuries, pts in respiratory difficulties, major blood loss


drug interactions: additive effects w/other CNS depressants, MAO inhibitors


dosage: Adult[0.5-1.0mg] SLOW IV push until desired effect, NOT for peds


notes: take vitals 2 min before and 2 mins after, IV/IO only, can cause vomiting/nausea, MCEP for non-cardiac pt

Ipratropium

indications: bronchial asthma, reversible bronchospasm associated w/chronic Bronchitis and emphysema l

Lidocaine

naloxone

Narcan


class: narcotic antagonist


indications: reversal of narcotic effects respiratory depression, unconscious of unknown etiology to rule out narcotic depression of CNS


contraindications: hypersensitivity, absence of indication


drug interactions: may induce narcotic withdrawal


dosage: Adult[0.4mg-2.0mg] IV/IO (total 2.0mg), [0.4-2.0mg] if IM/SQ titrate to respiratory rate, may repeat 2-3 mins, 2mg (1mg per nare) IN, Peds[0.1mg/kg] <5 y/o or <20kg [2mg] >5 y/o or >20kg may repeat if no response, Neonate same as peds (can mix 1 mL of naloxone and 9mL of D5W to get 0.04mg/mL)

Nitroglycerin

class: anti-anginal agent, vascular dilating agent


indications: chest pain, anginal pain


contraindications: hypersensitivity, severe HTN, pericardial temponade, intracranial pressure, hypovolemia, severe anemia


drug interactions: additive HTN w/beta adrenergic blocker, anti-HTN, calcium channel blockers, phenothiazines, tricyclic antidepressants, antihistamines, ED medication within 48 hrs


dosage: Adult[0.3-0.4mg] SQL 3-5 mins as needed three times, no peds use


notes: side effects of- headache, flushing, dizziness, burning under tongue, use only with MCEP if BP <100mmHg

Nitrous Oxide

class: inhalation anesthetic, gas


indications: used for pain management for: fractures, burns, kidney stones, musculoskeletal trauma, ABD pain, back pain, acute urinary retention


contraindications: altered LOC, head/chest injuries, intoxication, maxillofacial injuries, psychotic issues, COPD, <12 yrs or <75 lbs, respiratory distress, inner ear pain


side effects: dizziness, apnea, cyanosis, nausea/vomiting, Hypotension,


dosage: pt uses until they determine effectiveness using PT held demand valve

Oxygen

class: class 3 Gas, oxidizer


indications: suspected hypoxia, acute chest pain in which MI is suspected, shock, trauma, CO poisoning
