So patients may need to be on a stool softner, a laxative, or both if this is an issue w/ longterm opioid therapy.
Aripiprazole
Most Common: headache, agitation, insomnia or sedation, GI upset, weight gain [Consider switching to a different antipsychotic if weight gain exceeds 5% of baseline weight.]
Atenolol
Most Common: slow heart rate, hypotension, fatigue, drowsiness, GI upset (nausea, constipation, diarrhea)
Atorvastatin
Serious: muscle pain, tenderness, weakness, or brown urine (myopathy/rhabdomyolysis), jandice, i.e yellowing of the skin or eyes (liver damage); Common: diarrhea, nausea, indigestion, arthralgia, muscle pain, headache, insomnia, dizziness (all generally mild and typically disappear over time)
Azithromycin

Most Common: GI upset, xerostomia, either drowsiness or insomnia, profuse sweating. Sexual dysfunction can also occur (as impotence or ejaculation disorder), but it is reversible upon discontinuation.
Clonazepam
CNS depression (drowsiness, impaired coordination, memory impairment, depression), altered appetite (increased or decreased appetite), anticholinergic effects (xerostomia, dry eyes, constipation, difficulty urinating)
Clonidine

Fexofenadine
Side effects are rare and similar to those seen w/ placebo. The most common include headache, drowsiness, and throat irritation. Nausea and vomiting is seem more commonly in children 6 months to 5 years of age.
Fluconazole
Most Common: GI upset (nausea, vomiting), headache, rash. Although rare, angioedema and arrhythmias can occur.
Fluoxetine

Levothyroxine
Dosage too high leads to difficulty concentrating, fatigue, frequent bowel movements, goiter or thyroid nodules, heat intolerance, increased appetite, increases sweating, irregular menstrual periods, nervousness, restlessness, unintentional weight loss, fluttering or irregular heart beat, increased HR & BP, hair loss, and increased LFTs.

Most Common: application site reactions (blisters, bruising, irritation, etc.)

Meclizine
Most Common: drowsiness, thickening of bronchial secretions
Meloxicam
Most Common: GI upset (N/V/D/C, abdominal pain, heart burn), edema, headache or dizziness
Memantine
Most Common: dizziness, headache, confusion, GI upset, increased blood pressure
Metformin
Most Common: GI upset (diarrhea is experienced by up to 50% of patients taking metformin); Less Common: muscle weakness, headache, metallic taste, decreased B12 absorption. Side effects generally resolve over time.
Methylphenidate
Most Common: headache, insomnia, decreased appetite, nausea, irritability and viral infections [the last two are seen w/ longterm use in children]
Methylprednisolone
Most Common: headache, GI upset, sodium and fluid retention, insomnia
Metoprolol Succinate
CV: hypotension, bradycardia; CNS: dizziness, headache, depression, sedation; Others: rash, diarrhea
Metoprolol Tartrate
CV: hypotension, bradycardia; CNS: dizziness, headache, depression, sedation; Others: rash, diarrhea
Mometasone
Most Common: headache, cough, upper respiratory tract infections, nose bleed (epistaxis)
Montelukast
Headache, cough, dizziness, abdominal pain, upset stomach. In rare cases, leukotriene inhibitors can cause neuropsychiatric events, such as agitation or aggression, anxiousness, abnormal dreams or hallucinations, depression, insomnia, or suicidal ideation (tell patients to report any of these!)
Naproxen
Most Common: GI upset (N/V/D/C, abdominal pain, heart burn), edema, rash, itching, dizziness, nervousness, tinnitus (ringing/buzzing in the ear)
Niacin ER
Most Common: flushing, itching, GI upset (dyspepsia, nausea, abdominal pain)
Norgestimate & Ethinyl Estradiol
Most Common: GI upset (N, V, abdominal cramps, bloating), breakthrough bleeding or altered menstrual flow (this will usually subside within 3 months for most patients), breast tenderness, headache, weight gain (which is cyclic, usually gain weight around menses), hyperkalemia (rare)
Olanzapine
Most Common: drowsiness, extrapyramidal symptoms (pseudoparkinsonism, dystonic reactions, akathisia, tardive dyskinesia), headache, dizziness, weight gain, increased appetite, xerostomia, constipation, increased LFT (specifically ALT), muscle weakness, accidental injury [Most side effects are dose related, so the lowest effective dose should always be used.]
Olmesartan
Most Common: dizziness, headache, hyperglycemia, hypertriglyceridemia, diarrhea, hematuria (blood in the urine), flu-like symptoms

Headache, GI upset (abdominal pain, nausea, diarrhea), hypotension, photosensitivity, hypokalemia, anorexia

Most Common: drowsiness, dizziness, nausea, vomiting, constipation. [Most side effects decrease over time, except constipation. Therefore most patients will require a stool softner and/or laxative w/ longterm opioid therapy.]
Oxycodone/APAP
Most Common: CNS depression, respiratory depression, dysphoria or euphoria, sedation, GI upset (N/V/Constipation). As with other opoids, most side effects decrease over time, except constipation.
Pantoprazole
Most Common: headache, GI upset (abdominal pain, nausea, diarrhea)
Paroxetine
Most Common: either drowsiness or insomnia, headache, dizziness, decreased libido, GI upset (nausea, constipation, diarrhea), xerostomia, ejaculatory disturbances, muscle weakness or tremor, profuse sweating
Penicillin VK
Most Common: GI upset (N/V/D), oral candidiasis (thrush)

Potassium Chloride
Most Common: hyperkalemia, GI upset (abdominal pain, diarrhea, flatulence, n/v). [Taking each dose w/ plenty of fluid should decrease GI upset.]

Pregabalin

Propoxyphene-N/APAP
Most Common: CNS depression, euphoria or dysphoria, GI upset. [Note: Constipation is the one side effect of opioids that, if experienced, usually does not decrease over time.]
Quetiapine

Venlafaxine
Most Common: headache, dizziness, either drowsiness or insomnia, nausea, constipation, xerostomia, anorexia / weight loss, abnormal ejaculation, sweating
Verapamil
Most Common: hypotension, bradycardia, dizziness, headache, nausea
Vitamin D3
Generally well tolerated. Overdose can lead to Vitamin D toxicity, a.k.a. hypervitaminosis D, which may cause hypercalcemia. [Signs and symptoms include constipation and GI upset (nausea/vomiting), decreased appetite, muscle weakness, and cardiac arrhythmias.]
Warfarin
Common: bleeding or brusing more easily, GI upset (N/V/D), headache, dizziness; Serious: Severe or prolonged bleeding, skin or tissue necrosis, gangrene, purple toe syndrome
