Dobutamine (Dobutrex)
Action: stimulates Beta1 and increases cardiac contractility,

Class: Beta1- adrenergic

Dosage: IV Infusion 2.5-40 mcg/kg/min

Indications: Anaphylaxis; Cardiopulmonary arrest

Adverse effects: Angina; Hypertension; tachyarrhythmia
Dopamine (Intropin)
Action: At low dosages, can dilate blood vessels in brain, heart, kidneys to increase blood flow. High rates, it works on beta1 to increase cardiac contractility, and output.

Class: Beta1 -Adrenergic

Dosage: IV Infusion 1-50mcg/kg/min

Indications: Anaphylaxis, Cardiopulmonary arrest

Contraindications: Pts w/ catecholamine-secreting tumor known as a pheochromocytoma

Adverse Effects: Angina; HTN; Palpitations; tachyarrythmia

*Monitor hourly for IV site reaction*
Epinephrine (Adrenalin)
Action: At low dosages it works on Beta1, increasing cardiac contractility and herat rate. Also effects Beta2 and can be used in anaphylatic shock and acute asthma for broncho-dilation. At High dosages via iv infusion, it can be used to stimulate alpha 1 and increase vasoconstriction, which elevates BP.

Class: Alpha- and Beta- adrenergic

Dosages: Subcut- 0.1-0.5 mg 1/1000 concentration anaphylaxis

IV: 0.5-1 mg repeat 3-5 min
Fenoldopam (Corlopam)
Class: dopamine 1 agonist

Action: Induces arteriolar vasodilation through stimulation of D1 receptors. Used for treatment in severe hypertension and has benifits to renal function by increasing renal blood flow.

Available as a 10mg/ml injection

Dosage: IV 0.1-1.6 mcg/kg/min for up to 48 hours.

Indications: Hypertensive emergency in hospital setting.
midodrine (ProAmatine)
Class: Alpha1 -adrenergic

Action: Converted to active form desglymidodrine in the liver. stimulates alpha1 and causes both arterioles and veins to constrict, causing peripherial vasoconstriction.

Availability: 2.5- and 5-mg tablets

Dosage: 10 mg tid (q3-4h)

Indications: Orthostatic hypotension
