What are the indications for an Ace Inhibitor renogram?
To diagnose or exclude urinary tract obstruction, to assess renal function and urodynamics, to determine what percent of total renal function is contributed to each kidney, renal perfusion, and trauma.
What is the name of the drug used in conjunction with a renogram to diagnose or exclude renovascular hypertension?
Captopril
What preparation is necessary for this study?
Patient must be well hydrated, and should void before injection and after study is completed.
Any contraindications? If so, what are they?
No iodine contrast studies within 24 hours of exam, dehydration, pregnancy, breast feeding, no diuretics, and patient movement.
What type of collimator is used?
Low energy, high resolution (LEHR), parallel hole
What kind of images are obtained during this study?
Statics and dynamics
How long after the injection should imaging begin?
Immediately, static images @ 1, 3, 5, 10, 15, 20, 25, 30 minutes up to one hour.
How long does this study usually take to complete?
Approximately 1 hour
If a flow study is required, for how long should each image be obtained? How long for the dynamic images? Statics?
2 seconds/frame for 1 minute (flow), 30-60 seconds/frame, 20-30 minutes for dynamics, and 500,000 counts for statics.
How many images should be in a data set?
30 images (flow), 45-60 (dynamic)
Is there any special equipment used during this exam? If so, what are they?
yes; Blood pressure cuff
What radiopharmaceutical is used for a captopril study and what is the adult dose?
Tc-99m - DTPA, 5-15 mCi
What is another radiopharmaceutical that can be used for this study? What is the adult dose?
Tc-99m - MAG3, 5-10 mCi
How much Captopril (mg) is orally administered during the procedure?
25-50 mg
What is the mode of localization for Tc-99m - DTPA?
Glomerular Filtration
In what position is the patient situated before the study?
Supine
What views are taken of the patient's body?
Posterior
What are considered normal results for a Captopril study?
Normal blood pressure range (baseline), homogenous distribution in both kidneys, static images should be symmetrical, time-to-peak concentration is 3-5 minutes and clearance of Rph (T1/2) with 12-15 minutes,
What are considered abnormal results for a Captopril study?
Non-visualization of one or both kidneys, decreased activity, pyelonephritis, enlarged kidneys, Renal artery stenosis, hydronephrosis, elevated blood pressure
