Methotrexate
Cytotoxic drug causing early AB by blocking folic acid in fetal cells so they cannot divide.
Liquid form of A486
Advantage: can take early-not surgical (won't destroy or damage cervix)
Misoprostol (Cytotec)
prostaglandin analog that acts directly on the cervix causing it to dilate and soften and on the uterus causing contractions
Aspiration-mechanical vacuum or suction cutterage
most common procedure in 1st trimester; 95%; physician office clinic; 8-12 weeks since LMP; less than 5 minutes.
Actual cutterage procedure
speculum is inserted, cervix is anesthetized, dilated, canula connected to suction is insterted into uterus. Uterine cavity evacuated. Post-op care; provide juice and crackers
Methotrexate admin
IM or orally (usually in OJ)
Misoprostol vaginal placement
usually follows in 3-7 days; woman will return for follow-up visit to make sure pregnancy has been aborted
Side effects of medication abortions
Feel sick-fever and chills
Cramping, bleeding
May take 1-7 days
N/V/D
Dizziness (worse symptoms are caused by misoprostol)
Surgical side effects
Cramping
reverse lithotomy
may feel faint
give crackers and juice
should be able to walk to the recovery area
recover 1-3 hours
Complications of surgical procedure
Infection
Cervical stenosis
Cervical tear
Uterine puncture
Underestimated gestational age
Seizures
Retained tissue
Hemorrhage
Inability to complete abortion
