How is mucositis graded and what effect do the most severe grades of mucositis have on chemotherapy?
Mucositis is assigned an increasing grade from 1 to 4 based on severity. The more severe grades, 3 and 4, are grounds for limiting the doses of chemotherapy.
List five treatments for simple oral mucositis. What other treatment can help reduce the discomfort of mucositis?
1) Magic Mouthwash, 2) Salt Water Gargles, 3) Topical Analgesia with Xylocaine Viscous, 4) Systemic Analgesia, & 5) Nystatin. Treatment of heartburn with antacid, H2RAs, or PPIs can also reduce the discomfort of mucositis.
Describe four strategies for mucositis management.
1) Utilize magic mouthwash prior to meals, 2) Use patient controlled analgesia (especially prior to meals), 3) Minimize the number of oral medications the patient is taking, 4) Utilize alternative formulations, such as liquid formulations that are easier to swallow than tablets or transdermal delivery systems.
Name and describe the medication that is useful for reducing the severity of oral mucositis in patients having hematologic malignancies who are receiving myelotoxic therapy requiring hematopoetic stem cell support.
Palifermin (Kepivance): it is a recombinant human keratinocyte growth factor (KGF) that stimulates proliferation of epithelial cells.
What are the adverse effects of palifermin (Kepivance)?
Rash, Pruritis, Erythema
Describe dosing and administration of palifermin (Kepivance).
60 mcg/kg/day IV bolus for three consecutive days before and three consecutive days after myelotoxic therapy. It should not be administered for 24 hrs before, during, or for 24 hrs after after the infusion of myelotoxic chemotherapy.
