What does is mean to "Identify Hazards"?
Listing all hazards associated with each step in the operational analysis along with possible causes for those hazards.
What does it mean to "Assess Hazards"?
For each hazard identified, determine the degree of risk in terms of probability and severity .
What does it mean to "Make Risk Decisions"?
Develop risk control options, start with the most severe risk and select controls that can reduce the risk to a minimum and decide if the benefits outweighs the risk. If the risk outweighs the benefits or assistance is required communicate with higher ups in the chain of command.
What does it mean to "Implement Controls"?
Engineering Controls: Reduce risk by design, material selection, or substitution when technically or economically feasible.

Administrative Controls: Reduce risk by providing warnings, markings, placards, signs, and notices.

Wearing PPE; this serves as a barrier between personnel and hazards.
What does it mean to "Supervise"?
Doing follow-up evaluations to ensure the controls remain in place and have the desired effect. Monitor for changes, which may require more ORM, and take corrective actions when necessary.
Why do we Tag-out equipment?
Provides personnel and ship safety and prevent damage to equipment.
Prevent improper operation when equipment is in abnormal condition.
Provides procedure for use when an instrument is unreliable.
What are 5 things a person hanging a Tag-out must ensure?
-The correct component is tagged.
-The component is as specified on the tag and TORS(Tag-Out Record Sheet)
-Obtain authorizing officer permission for re-positioning components.
-Verify and confirm valve position IAW TUM(Tag-out User's Manual.)
-Post tags in plain sight and so they are apparent to anyone who may attempt to operate.
After a tag is posted, the person posting the tag shall sign and initial what blocks?
Sign block 4 of the tag.
Initial block 17 of TORS.
After a tag is complete the poster must present the TORS to a second qualified person for what reason?
So they can independently verify the tags were posted correctly. (The checker)
Is the poster allowed to accompany the checker?
No
After checking the component the checker shall sign and initial which blocks?
Sign block 5 of the Tag.

What is the color and purpose of a "Caution" tag?
Yellow.
Provides temporary instruction on use of equipment or unusual actions that must be done to operate.
(Caution tags are not used if equipment or personnel can be harmed while operating. (Danger tags will be.)
What is the color and purpose of a "Danger" tag?
Red.
Prohibit operation or removal of equipment.
What is the color and purpose of an "Out-of-Commission" label?
Red.
To inform when instruments will not correctly indicate parameters or readings because they are defective. This indicates the instrument cannot be relied on and must be repaired and re-calibrated.
What is the color and purpose of an "Out-of-Calibration" label?
Orange.
Identify instruments that wont accurately indicate parameters or readings.
What do you do when you notice a missing or damaged tag?
Immediately report to the authorizing officer.

Checks and balances system to prevent equipment from degrading, by conducting preventive and corrective maintenance.
