DD 1348-1A

Used to turn in material to DRMO (such as old desk, chairs, computer, etc.)

Used by S-1, S-6, and S-8 as a storeroom issue document.

DD FORM 200

Used as a shipping document for material to and from outside activities.

Used by contracting agent to procure non-NSN items that are over $2,999.99 for material and over $2,499.99 for services.

SF 368

This is known as the Quality Deficiency Report (QDR). Used when you receive material off station and the item is broken or for material received that is the wrong material.

Material must be over $2,500.00 in order to submit ODR. Report is sent to the shipper to request a replacement shipment or to receive credit for material.

SF 364

This is known as a Report of Discrepancy (ROD). When material is shipped but not received by command after 30/90 days while on deployment.

NAVSUP 1371

Fleet COSAL Feed Back Report.

This form is used when there is an error in the Allowance Parts List (APL) or Allowance Equipage List (AEL).

NAVSUP FORM 1220-2

Allowance Change Request (ACR) Form. This form is used to request a higher quantity than what is prescribed by the Quantity Per Application (QPA).

Only one NSN is allowed per ACR.

FEDLOG

Used to identify special characteristics of material that is required.


Ex: Hazardous material can be identified by looking for the SMCC and HMIC, DLRs/FLRs can be identified by the COG and MCC, Gold Disc material can be identified by the SMIC and LMC.
