What are the 3 points on the Jugular venous pulsation curve to know?
-A
-C -V |
What is the a wave?
Atrial contraction
What is the c wave?
Right v contraction - shows up when the tricupsid valve bulges into the atrium.
What is the v wave?
Increasing atrial pressure due to filling against the closed tricuspid valve.
What does S1 heart sound coincide with on the ECG? What is this?
The R wave - this is closing of the mitral/tricuspid AV valves at the START of isovolumetric contraction of the ventricles.
What is S2 and where does it coincide with on the ECG?
S2 = shutting of the aortic and pulmonic valves; seen on ECG as the end of the T wave, After all ejection
When is S3 heard?
At the end of rapid ventriculr filling during phase 4 of the cardiac cycle
What disease is S3 associated with?
Congestive heart failure
When is S4 heard? What is is caused by?
Just before the QRS complex - caused by a stiff ventricle or high atrial pressure
What is s4 also called and what is it associated with?
-Atrial KICK
-ASsociated with a hypertrophic ventricle |
What heartsound normally splits?
S2
What is S2 again?
Shutting of the aortic/pulm valves
What physiologically causes S2 to split?
Inspiration
What will cause WIDE splitting?
Pulmonic stenosis
What will cause Fixed splitting?
Atrial septal defects!
What is paradoxical splitting?
When the pulmonic valve shuts BEFORE the aortic during the split
Which valve shuts first in S2 split normally?
Aortic
What can cause paradoxical splitting?
Aortic stenosis
And what can cause S3?
CHF
And what can cause S4?
Ventricular hypertrophy (requiring an aortic kick)
