What is the most frequent initial rhythm in out-of-hospital witnessed cardiac arrest?

Vfib

What is the correlation between survival rates and 1 minute delays before defib?
7-10% decrease for every minute
What is the likely outcome of CPR in a patient with VF (ROSC, etc.)?
-prolongs VF for 5-10 min and delays asystole
-very unlikely to terminate VF and restore a perfusing rhythm
AED or compressions first?
-right away: compressions > AED (need good flow for AED to work right) but keep time to AED < 3 min
-delayed (e.g. when EMS gets to scene): AED > compression
Where should defibrillator pads be placed in a patient with a pacemaker or ICD?
avoid placing the pads over the implant; anteroposterior or anterolatleral locations are both acceptable
What should be done with med patches when patients require defibrillation?
avoid placing pads directly on top of patches (less effective shock; can cause burns); if time, remove patch and wipe clean
What should be done if a patient requiring defibrillation is wet?
reasonable to try to dry the patient or at least where the pads will be applied; not a contraindication to using AED; make sure rescuers are clear from the water
What should be done if a patient requiring defibrillation is hairy?
rip off hair with an electrode pad, or quickly shave the chest
