What starts off the Post-Cardiac Arrest Care Algorithm?

What is the first step in Post-CA care?
Ensure adequete ventilation and oxygenation
How do you ensure adequete oxygenation?
By monitoring continuos wave capnography to make sure it is at least 35-40 mm Hg, and titrating FiO2 so that SpO2 is at least 94%
What breath rate should you start patients on for Post-CA care?
10-12 breaths per min
What step comes after ensuring adequete oxygenation and ventilation?
Treat hypotension <90 SBP
How do you treat hypotension?
Give IVF bolus of 1-2 L NS or LR - chilled to 4'C if you're going to induce hypothermia
What drugs can you use to treat hypotension?
Epi
Norepi
Dopamine
What dose of Epi or Norepi do you give for HYPOTENSION?
0.1 to 0.5 mcg/kg/min
What dose of dopamine do you give for hypotension?
5-10 mcg/kg/min
What should you titrate all infusions for hypotension to achieve?
SBP of 90 or greater
MAP of 65 or greater
What should you do in the Post-CA Care algorithm after treating hypotension?
Check ability to follow commands
What should you do if the patient does not follow commands?
Consider induced hypothermia to 32-34'C for 12-24 hr
What should you do if the patient DOES follow commands, or after inducing hypothermia for those that don't?
Get a 12-lead ECG to determine STEMI or high suspicion of AMI
What if there was a STEMI or AMI?
Send to the cath lab for coronary reperfusion
What is there was not a STEMI or AMI?
Send to the ICU
