What is Amantadine used for?
Influenza A prophylaxis
When does Amantadine have to be given if you want it to have a therapeutic effect on Influenza A? What effect will that be?
Within the 1st 2 days of symptom onset; will reduce fever in 50% of patients and shorten illness duration by 1-2 days
What is Amantadine's mechanism?
Blocks viral uncoating by interfering with Infl A M2 protein (ion channel)
How is Amantadine administered? How is it eliminated?
Oral administration
Eliminated unchanged by kidneys
So what do you have to be careful of when giving Amantadine?
Renal disease - would need to lower the dose
Though Amantadine is generally well tolerated, what are its main side effects?
-CNS side effects - Headache, anxiety..
-Nausea/vomiting
What is the mechanism of action of Oseltamivir?
Prodrug competitive inhibitor of influenza's neuraminidases.
What does inhibition of neuraminidase result in?
Inhibition of viral release from infected cells and viral penetration into resp epithelium
Where is Oseltamivir converted from its prodrug form to the active form?
In the liver by esterases
What are the 2 uses of Oseltamivir?
-Treatment of uncomplicated Influenza A/B in children 1 yr or older if given in 48 hrs of onset
-Prophylaxis in patients 1yr old or more
How is Oseltamavir administered?
Orally
How is Oseltamavir eliminated?
By the kidney
What are 3 side effects of Oseltamivir?
-Bronchitis!!!!
-Nausea/vomiting
