Cleft lip is due to failure of what fusion process?

Fusion of nasal prominence and maxillary prominence

Cleft palate is due to the failure of what fusion process?

palatal processes to fuse with one another at the pre-maxilla and nasal septum

What are the two types of cleft lip?

Complete (continues into the nose)

Incomplete

How do we identify a submucosa cleft palate?

fused mucusal membrane beneath it

bifid uvula

palpable notch hard palate where the two processes failed to fuse

What is a cleft soft palate?

incomplete fusion of the soft palate

What is cleft hard palate?

incomplete fusion of hard and soft palate

can be unilateral or bilateral

How does cleft palate cause eustachian tube dysfunction?

when swallowing the hard palate elevates and it pulls down on tensor veil palatine muscle in a pulley system which causes opening of the tube

In this case the pulley system is MESSED UP so and so the eustachian tube is not easily opened

What are some repercussions of a dysfunctional Eustachian tube formation?

  • non equalization of pressure in middle ear
  • creates a negative pressure
  • negative pressure draws fluid out of mucosa = fluid in middle ear
  • this fluid impairs hearing and increases risk of ear infection

Will cleft lip palate children eventually outgrow the chronic otitis media?

Yes

What are some problems with dentition and cleft palate

missing teeth

supernumerary teeth

malformed teeth

mal-positioned teeth

cross bite

What is the most affected tooth by cleft palate?

lateral incisor followed by....

canine

central incisor

Does cleft palate affect speech? If so how?

Yes

velopharyngeal insufficiency (inability to fully close nasopharynx with soft palate causing air to escape)

hypernasality detected
