The first diagnosis that I chose for Cathy was Substance Use Disorder: Cannabis Use Disorder, Tobacco Use Disorder, and Alprazolam Use Disorder. According to Shmulewitz, Greene, and Hasin (2015), “substance abuse disorders are diagnosed by cognitive, behavioral, and physiological symptoms that accompany repeated substance use despite the occurrence of clinically significant problems” (pg. 1878). DSM-5 gives specific criteria that must be met in order to meet the diagnosis for Substance Use Disorders. According to DSM-5, Substance Use Disorders are characterized by “impaired control, social impairment, risky use, and pharmacological criteria” (pg. 483). Cathy reported that she has been having suicidal ideations in the past week and that she has a vague plan that includes drinking bleach. In addition, Cathy reported a history of psychiatric hospitalizations and suicide attempts by overdose. Therefore, Cathy meets DSM-5 criteria for Borderline Personality Disorder that states individuals with Borderline Personality Disorder may have “recurrent suicidal behaviors, gestures, or threats, or self-mutilating behavior” (pg. 663). Lastly, Cathy reported that she does not like to be alone and that she thinks she is depressed because of feelings of emptiness. According to Hersh (2013), safety should be the key concern when working with individuals with borderline personality disorder. This involves “repeated, direct, explicit discussions of a student’s suicidal thoughts, plans, or behaviors may be required, as well as investigation of other potentially self-destructive behaviors” (pg. 312). Furthermore, setting up a safety plan that Cathy and Beth both agree to implement may also help to ensure Cathy’s safety. The second issue that I would address working with Cathy is investigating other potentially self-destructive behaviors; which for Cathy includes her severe substance abuse and cutting.
