Use of Group Based Trajectory Models to Identify Sociodemographic and Clinical Predictors of Variable Statin Adherence Patterns among Patients Enrolled in a Medicare Advantage Plan
Prescription medication use is on the rise in the United States, especially among older adults. Nearly 88% of community dwelling older adults were on at least one prescription medication between 2010-2011 (PMC5024734). Consequently, the annual spending on prescription drugs in US continues to increase, amounting to $259 billion (PMC3934668) in 2010. Adherence is defined as the extent to which a patient’s medication taking behavior …show more content…
Statins are the most commonly prescribed lipid lowering agents8 with extensive clinical trial data demonstrating benefits in primary and secondary prevention of cardiovascular disease (CVD) and reduction of cardiovascular mortality.9-14 Despite the well documented efficacy, adherence to statins remains suboptimal. 8, 15 Studies report high discontinuation rates15-17 with 50% of patients discontinuing within one year, and more over time,8, 15-17 especially among elderly patients.15, 18, …show more content…
This method creates groups of patients with similar adherence patterns over time using pharmacy refill data and can better depict the longitudinal adherence behavior. Patients within each trajectory have a similar adherence pattern and therefore may have similar characteristics.
The objective of these study was to identify trajectories of adherence to statins over a one year period among patients enrolled in a Medicare Advantage Plan in Texas and to identify the clinical and sociodemographic predictors of falling into each trajectory. Understanding characteristics of patients that put them at risk for nonadherence has the potential to inform how best to intervene. This can instruct the future development of protocols and tools to better individualize interventions for these
