Summary of Pertinent Case Features
Presenting Problem: Patient states she has always been a tense person, has experienced agoraphobic symptoms off and on again during the last seven years, but the intensity has increased substantially in the last six months. Symptoms include rapid or irregular heartbeats (similar to having a heart attack), crying, and moaning. Patient also claims to suffer feelings of helplessness and depression. Patient reports that she was stable in her functioning, only showing occasional signs of nervous or anxiety in the early years of her marriage. She was brought to the mental health center by her daughter, who claimed that the patient was driving her crazy with requests that she accompany her everywhere.
Medical History: Patient reports that she has been suffering from a “heart disease” for the last four years. Patient’s physician has often reassured her that there is no pathology to Patient’s heart palpitations and their irregularity. She sought treatment for rapid or irregular heartbeats and was subsequently diagnosed the patient with anxiety and tension, for which the physician prescribed regular exercise and tranquilizers for any severe episodes of anxiety. Patient reports a past miscarriage after birth of her daughter.

A snow storm made traveling difficult, leaving Patient feeling isolated. Patient tried calling her sister, but the line was dead. Patient’s husband works as a wales representative and subsequently spends much of his time on the road. His absence has been a constant stressor, exacerbated by the event at the Patient’s daughter’s house. Patient admits to relaxing considerably when her husband is home.
