BV vs. Trich vs. Candida |
- BV: fishy, thin discharge, no inflammation, clue cells, amine odor with KOH, pH> 4.5 -> metronidazole or clinda - Trich: thin yellow-green, frothy discharge, inflamed vagina, pH > 4.5 with motile on wet mount -> metronidazole treat partner too - Candida: thick discharge with vaginal inflammation, normal pH, pseudohyphae -> fluconazole |
Spontaneous Abortion vs. Intrauterine Fetal Demise |
SAB = < 20 wk IUFD = > 20 wk |

- fasting < 95, 2 hr postprandial < 120 - all pregnant women should be screened btw 24-28 wks - insulin doesn't cross the placenta |
Maternal Hyperglycemia |
- increased risk of congenital heart disease, NTD, spontaneous abortion - hyperglycemia -> macrosomia -> organomegaly and increased O2 demand and polycythemia - shoulder dystocia an fetal hypoglycemia are birth complications |
Milk production |
- sudden loss of E2 and progesterone = increased prolactin - prolactin = milk production, oxytocin = milk let down |
condyloma acuminatum vs latum |
- HPV = condyloma acuminatum, cleared with chloroacetic acid - syphillis = condyloma latum, in secondary syphilis -> flat, velvety lesions |

Lichen Sclerosis |
- autoimmune etiology - pruritus, dyspareunia, dysuria, white atrophic paper-like skin, obliteration of labia minora, decreased size of introitus - increased risk for vulvar squamous cell carcinoma -> punch biopsy - topical corticosteroids |
Lichen planus |
- purple plaques or papules, wickhams striae, - |
