In stable angina, the artery occluded may still allow enough blood flow to satisfy the oxygen demands at rest. However, with increasingly demanding activities, supply will be insufficient.
This is why pain usually occurs during exertion. |

In high output, the heart is still pumping out a significant amount of blood, but still inadequate due to excessive metabolic demands of the body.
