On 1/8/2016 SC met with Pa and PAS/son in Pa's home for home visit. Pa appeared properly groomed and dressed in street clothes. The home was unclean, and cluttered with urine odor nevertheless, all utilities appeared to all be in working order. Pa reported no, falls, hospitalization, change in health status, or medication.
