    Ms. Martin is a twenty-one year old Caucasian female, single has no children was referred to Evolutions Treatment facility for her Substance abuse usage. She reported being born in Staten Island, New York and raised in Jackson, New Jersey. She was primarily raised by her biological parents. She stated she relocated to Port St. Lucia, Florida one and half years ago for her substance abuse problems. Ms. Martin resides in West Palm Beach, Florida with her boyfriend. The client described her childhood as "good," and "healthy" and opined it as a normal family. She stated she was not deprived of anything while growing up. She reported her relationship with her mother was "good", and father the same. She further related that she and her siblings also got along within the home. Ms. Martin noted she feels closet to her mother because she can express her feelings to her. Ms. Martin also mentioned being in a relationship with her boyfriend for one and half years. She stated they have a good bond. In regards to sexual abuse history, the client denied ever witnessing or being involved in any trauma related incidents. In regards to her drug use, Ms. Martin reported she started using drugs when she was sixteen years old. She noted while in high school she was first introduced to "heroin", which she used often.
