All postive concordance or all negative across precordial leads suggests VT
Fusion beat - suggests VT
Capture beats - suggests VT
Rabbit ears
RSR’ complexes with a taller left rabbit ear. This is the most specific finding in favour of VT
Morphology
If not typical of RBBB or LBBB, likely VT
Axis
Extreme axis deviation ("Northwest") likely VT
Definition of sustained vs. non-sustained VT
Greater than or less than 30 sec
Duration
>0.14 sec likely VT
Brugada criteria
If any are true, dx VT: 1) absence of RS complex in any precordial leads (100% specific for VT)
2) if RS present, at least one RS interval (start of R wave to deepest part of S wave) > 0.1 sec
3) A-V dissociation
4) Morphology criteria for VT in V1 and V6 both
Other important considerations in VT
TCA OD, hyperkalemia, Dig toxicity
Each has it's own unique tx that must be applied for VT to resolve
