Where does the trigeminal nerve originate? Where does it go?
From the pons (middle portion of the brain stem), and passes anteriorly to enter the trigeminal cave.
Where is the trigeminal cave? What does it contain?
-Trigeminal cave is located in the middle cranial fossa and surrounded by dura.
-Contains the semilunar (sensory) ganglion of the trigeminal nerve.
What 3 sensory divisions arise from the semilunar ganglion?
-ophthalmic (CN V1)
-maxillary (CN V2)
-mandibular (CN V3); has motor component
-ophthalmic (CN V1)
-maxillary (CN V2)
-mandibular (CN V3); has motor component
What happens if the dura mater is irritated?
The meningeal branches sense the irritation, and it is felt as a headache.
The meningeal branches sense the irritation, and it is felt as a headache.
What is the ophthalmic division (CN V1) of the trigeminal nerve?
The superior and smallest of the 3, ALL SENSORY. Supplies the:
-eyeball
-lacrimal gland
-conjunctiva (inner eyelid)
-upper eyelids
-part of the nasal mucosa
-skin of the nose
-forehead and part of scalp
The superior and smallest of the 3, ALL SENSORY. Supplies the:
-eyeball
-lacrimal gland
-conjunctiva (inner eyelid)
-upper eyelids
-part of the nasal mucosa
-skin of the nose
-forehead and part of scalp
What does the supraorbital branch of the ophthalmic nerve do?
Exits via the supraorbital notch, or foramen, to innervate the upper eyelid and its associated conjunctiva. Also innervates the scalp anterior to the vertex.
What would happen if the supraorbital nerve is cut?
Sensation of that side is gone from the upper eyelid and the entire anterior scalp. Takes a long time to heal.
What is the maxillary division (CN V2) of the trigeminal nerve?
ENTIRELY SENSORY, has branches that innervate the:
-nose
-skin and mucosa of cheek
-upper lip
-upper teeth (maxillary teeth)
-maxillary palate
What branches of the maxillary division (CN V2) are in the infraorbital canal?
-superior alveolar nerves
-infraorbital nerve
What do the superior alveolar nerves of the maxillary division (CN V2) supply?
The maxillary premolars, canine and incisor teeth.
Why are multiple injections of anesthetic used when getting dental work done on upper teeth?
To provide anesthesia for both superior alveolar nerves, so no pain is felt in the upper teeth.
What does the infraorbital nerve of the maxillary division (CN V2) supply?
-lower eyelids
-lateral nose
-upper lip
What is the mandibular division (CN V3) of the trigeminal nerve?
Contains both sensory AND MOTOR fibers. Each set of fibers supply different areas.
What do the sensory fibers of the mandibular division (CN V3) supply?
-mandibular teeth and gingiva
-skin of the temporal region
-part of the auricle of the ear
-lower lip
-lower part of face
-anterior 2/3rds of tongue
What doe the motor fibers of the mandibular division (CN V3) supply?
-muscles of mastication
-tensor muscles of the palate and tympanic membrane
-mylohyoid
-anterior belly of digastric
What are the branches of the posterior division of the mandibular division (CN V3) of the trigeminal nerve?
-auriculotemporal n.
-lingual n.
-inferior alveolar n.
What does the auriculotemporal nerve of the mandibular division (CN V3) do?
Provides SENSORY fibers to the upper part of the auricle of the ear and external auditory meatus, lateral surface of tympanic membrane.
Carries MOTOR fibers (postganglionic parasympathetic) from the otic ganglion to the parotid gland
What does the lingual nerve of the mandibular division (CN V3) do?
MOTOR: Joined by the chorda tympani nerve (branch of facial nerve) which contains preganglionic parasympathetic fibers, innervates the submandibular and sublingual glands.
SENSORY: chorda tympani nerve has fibers for taste from the anterior 2/3rds of the tongue and adjacent gingiva.
What is inferior alveolar nerve of the mandibular division (CN V3) do?
Motor portion - innervates mylohyoid and anterior belly of digastric
Sensory portions:
-in mandibular canal, provides sensory innv to teeth and gingiva of lower jaw
-mental nerve
Where does the mental nerve come from? What does the mental nerve do?
It is the terminal portion of the inferior alveolar nerve. Exits via the mental foramen and provides sensory innervation to the skin of the chin and lower lip.
What is Shingles?
A virus associated with the trigeminal nerve. Can affect all areas of trigeminal n. stimulation.
