If your train develops a fault, where would be deemed a suitable location to stop to deal with the fault. |
Stop at a station, a signa, clear off junction points to allow the signaller to run other trains past you whilst you attempt to rectify the fault. |

Avoid tunnels, viaducts, deep cuttings, over points or within level crossing controls |

What is the gtr fix or fail policy |
The objective is to get the train moving again or declare it a failure and request assistance ASAP to reduce delays to other trains |

Explain the 4 steps to fix or fail |
Stop in a safe and convenient location Inform signaller with defect within 2 minutes Inform customers of delay via PA Carry out basic cab checks. Check DRA,traction interlock, line light, VCB light, main air pressure, cab mcb’s, DMI, Mitrac, orange bils, try pan up reset if possible. |

If the fault is rectified, what must driver still do |
Submit a report to or phone fleet control at the next convenient point without causing further delays. |

What if initial checks prove unsuccessful |
Contact fleet control at three bridges immediately once signaller has been informed |

What will happen after 5 minutes if failure to rectify fault |
ROC will decide whether the 5 minute fix or fail policy will be implemented or whether fault investigation should continue. If still unable to fix a fitter may be sent out or failing that an assisting train to aid recovery. |

What must you do before walking/ examining the outside of your train? |
Ask the signaller to block adjacent lines and each a clear understanding as to which lines have been blocked and which remain open. |

If you are happy that the details are correct you should then |
Repeat your name/grade Location speaking from The reference number you were given. |

If you require an assisting train what must you do |
Contact the signaller ASAP. Agree what kind of assistance and from which direction Make sure thensignaller understands the problem, location and exactly what the assisting driver is required to do. |

What must you do once assisting train has been requested |
Secure train. Keys out, brake into emergency, cab door locked. Lay down any protection needed Check tail lights are illuminated if assistance is from rear Check a white light is displayed if coming from the front |

How far from failed train should detonators be placed |
300 metres and 20 m apart in the direction the failed train will come from |

When can you lay detonators before the 300m distance |
At a signal applying in that direction with red flag The 300 metres fall within a tunnel in which case proceed to far end of tunnel and wait with red flag |

What should you do in deciding which direction to protect first if no immediate communication can be made with signaller. |
Lay three detonators in direction that gives you quickest means of contacting signaller. Ie where you know a SPT is located. If signaller says assisting train will come from other direction pick up detonators and make way to other side agreeing a time frame with signaller for getting to protection distance. |

What should you do if your train fails in a TBW or ESW area |
Carry out assistance protection to rear placing three detonators |

If you are the assisting driver the signaller will tell you |
Location of failed train How it is protected Where the driver will meet you Where the failed train is to be taken When you may PSAD protecting them obstructed line |

What must you do in approach towards the failed train |
Keep a sharp lookout Only enter a tunnel if you have picked up the failed driver or you have been told the tunnel is clear and driver is not in there. You may have to walk through tunnel to establish this. |

If you are the driver of failed train you must |
Get in cab of assisting train Tell driver the location and protection laid down Instruct driver to proceed towards the protection point Stop on exploding the detonators Proceed at extreme caution. Ensure the driver gets authority to PSAD before proceeding into failed train |
