Hydrocodone/APAP
CNS: CNS depression (bradycardia, hypotension), dizziness, drowsiness, dysphoria, euphoria; GI: constipation (does not go away over time), nausea, vomiting, heartburn, xerostomia; Hematologic: Agranulocytosis, Prolonged Bleeding Time, Thrombocytopenia; Others: Itching & Rash, Urinary Retention, Liver or Kidney Damage, Hearing Loss, and Dose-Related Respirator Depression
Atorvastatin
Serious: muscle pain, tenderness, weakness, or brown urine (myopathy/rhabdomyolysis), jandice, i.e yellowing of the skin or eyes (liver damage); Common: diarrhea, nausea, indigestion, arthralgia, muscle pain, headache, insomnia, dizziness (all generally mild and typically disappear over time)
Levothyroxine
Dosage too high leads to difficulty concentrating, fatigue, frequent bowel movements, goiter or thyroid nodules, heat intolerance, increased appetite, increases sweating, irregular menstrual periods, nervousness, restlessness, unintentional weight loss, fluttering or irregular heart beat, increased HR & BP, hair loss, and increased LFTs.
Lisinopril

CV: hypotension, bradycardia; CNS: dizziness, headache, depression, sedation; Others: rash, diarrhea
Montelukast
Headache, cough, dizziness, abdominal pain, upset stomach. In rare cases, leukotriene inhibitors can cause neuropsychiatric events, such as agitation or aggression, anxiousness, abnormal dreams or hallucinations, depression, insomnia, or suicidal ideation (tell patients to report any of these!)
Escitalopram
Headache, either drowsiness or insomnia, nausea, sexual dysfunction (ejaculation disorder)
Albuterol Inhaler
Serious: paradoxical bronchospasms, CNS excitation (angina, HTN, MI, arrhythmias), hypokalemia; Common: throat irritation and cough, dizziness, nausea, headache, irregular or rapid heart rate.
Simvastatin
Serious: muscle pain, tenderness, weakness, or brown urine (myopathy/rhabdomyolysis), jandice, i.e. yellowing of the skin or eyes (liver damage); Common: abdominal pain, nausea, constipation, headache, upper respiratory tract infections
Amlodipine

Metformin
Most Common: GI upset (diarrhea is experienced by up to 50% of patients taking metformin); Less Common: muscle weakness, headache, metallic taste, decreased B12 absorption. Side effects generally resolve over time.
Metoprolol Succinate
CV: hypotension, bradycardia; CNS: dizziness, headache, depression, sedation; Others: rash, diarrhea

Hypotension, photosensitivity, hypokalemia, anorexia
Rosuvastatin

Furosemide

Common: bleeding or brusing more easily, GI upset (N/V/D), headache, dizziness; Serious: Severe or prolonged bleeding, skin or tissue necrosis, gangrene, purple toe syndrome
Fluticasone & Salmeterol
Most Common: headache, dizziness, upper respiratory tract infections, throat irritation, GI upset, oral candidiasis (thrush); Serious: CNS excitation leading to rapid or irregular heart beat, hypertension, or even MI. Hypothyroidism and abnormal liver function test are also possible.
Ibuprofen
Most Common: nausea, vomiting, diarrhea, constipation, abdominal pain, heartburn; Less Common: edema, rash or itching, dizziness, nervousness, tinnitus
Sertraline
Most Common: dizziness, headache, either insomnia or drowsiness, GI upset, xerostomia, tremors, decreased libido and ejaculatory disorders, sweating
Valsartan
Most Common: dizziness, headache, > %50 increase in BUN; Less Common: hypotension, hyperkalemia
Zolpidem
Most Common: headache, nausea, dizziness, drowsiness; Less Common: depression, xerostomia, GI upset
Oxycodone/APAP
Most Common: CNS depression, respiratory depression, dysphoria or euphoria, sedation, GI upset (N/V/Constipation). As with other opoids, most side effects decrease over time, except constipation.
Duloxetine
Most Common: either drowsiness or insomnia, headache, dizziness or fatigue, nausea/constipation/diarrhea, xerostomia, decreased appetite
Quetiapine
Most Common: increased blood pressure (in children and adolescents), drowsiness, headache, hyperglycemia, increased cholesterol, weight gain, xerostomia, constipation, postural hypotension
Venlafaxine
Most Common: headache, dizziness, either drowsiness or insomnia, nausea, constipation, xerostomia, anorexia / weight loss, abnormal ejaculation, sweating
Tamsulosin
Most Common: headache, dizziness, abnormal ejaculation, postural hypotension, rhinitis
Alprazolam
Most Common: CNS depression, drowsiness, impaired coordination, memory impairment, depression, altered appetite (increased or decreased), anticholinergic effects (xerostomia, dry eyes, blurred vision, constipation, difficulty urinating, cognitive impairment).

Most Common: increased BUN and serum creatinine, dizziness and hypotension, hypokalemia, diarrhea

Alendronate

Drospirenone & Ethinyl Estradiol
Most Common: GI upset, abdominal cramps, bloating, breakthrough bleeding or altered menstral flow, breast tenderness, headache, weight gain, hyperkalemia
Vitamin D3
Generally well tolerated. Overdose can lead to Vitamin D toxicity, a.k.a. hypervitaminosis D, which may cause hypercalcemia. [Signs and symptoms include constipation and GI upset (nausea/vomiting), decreased appetite, muscle weakness, and cardiac arrhythmias.]
Ezetimibe
Most Common: diarrhea, fatigue, arthralgia, upper respiratory tract infections
Gabapentin
Most Common: CNS depression, peripheral edema, GI upset
Pregabalin
Most Common: peripheral edema, CNS depression, weight gain, xerostomia, blurred vision
Sildenafil
Most Common: headache, dyspepsia, flushing, insomnia
APAP/Codeine
Most Common: CNS depression (dizziness, sedation), GI upset (constipation, nausea, vomiting), shortness of breath. Note: Constipation is the one side effect that does not go away over time. So patients may need to be on a stool softner, a laxative, or both if this is an issue w/ longterm opioid therapy.
Fluconazole
Most Common: GI upset (nausea, vomiting), headache, rash. Although rare, angioedema and arrhythmias can occur.
Intranasal Fluticasone
Most Common: headache, dizziness, nasal irritation
Losartan
Most Common: diarrhea, chest pain, fatigue, hypoglycemia, nasal congestion, cough [Note: Most of these side effects seem to be seen more in patients w/ diabetic nephropathy].
Propoxyphene-N/APAP
Most Common: CNS depression, euphoria or dysphoria, GI upset. [Note: Constipation is the one side effect of opioids that, if experienced, usually does not decrease over time.]
Valacyclovir
Most Common: headache, GI upset; Can Cause: neutropenia, increased LFTs, nasopharyngitis
Lorazepam
Most Common: CNS depression (drowsiness, impaired coordination, memory impairment, weakness, depression)
Pantoprazole
Most Common: headache, GI upset (abdominal pain, nausea, diarrhea)
Amlodipine/Benazepril
Most Common: hypotension, headache, cough, GI upset
Conjugated Estrogens
Most Common: headache, breast pain, abdominal pain, back pain, vaginal hemorrhage
Oseltamivir
Most Common: GI upset (N/V/D and abdominal pain). Usually subsides after 2 days of treatment and taking w/ food may help.
Tiotropium
Most Common: xerostomia, upper respiratory tract infection, pharyngitis, sinusitis
Aripiprazole
Most Common: headache, agitation, insomnia or sedation, GI upset, weight gain [Consider switching to a different antipsychotic if weight gain exceeds 5% of baseline weight.]
Methylphenidate
Most Common: headache, insomnia, decreased appetite, nausea, irritability and viral infections [the last two are seen w/ longterm use in children]
Digoxin
Most Common: GI upset, anorexia, blurred vision, headache, bradycardia
Fluticasone
Most Common: headache, dizziness, nasal irritation
Fexofenadine
Side effects are rare and similar to those seen w/ placebo. The most common include headache, drowsiness, and throat irritation. Nausea and vomiting is seem more commonly in children 6 months to 5 years of age.
Olmesartan
Most Common: dizziness, headache, hyperglycemia, hypertriglyceridemia, diarrhea, hematuria (blood in the urine), flu-like symptoms
Pravastatin
Most Common: headache, fatigue, rash, GI upset
Triamterene/Hydrochlorothiazide
Most Common: electrolyte disturbances, fatigue, muscle cramps, GI upset, fast heart rate, hypotension
Methylprednisolone
Most Common: headache, GI upset, sodium and fluid retention, insomnia
Cyclobenzaprine
Most Common: drowsiness, dizziness, xerostomia
Clonidine
Most Common: xerostomia; May Also See: drowsiness, dizziness, headache, edema, constipation, impotence (which will resolve once clonidine is discontinued)
Risedronate
Side effect profile is similar to that of placebo if taken as directed. The most common include increased blood pressure, headache, GI upset, and back pain.
Diazepam
Most Common: drowsiness, ataxia (loss of muscle coordination), fatigue
Carisoprodol
Most Common: drowsiness, dizziness, headache, GI upset
Promethazine
Most Common: sedation, xerostomia, dizziness, blurred vision, GI upset
Memantine
Most Common: dizziness, headache, confusion, GI upset, increased blood pressure
Potassium Chloride
Most Common: hyperkalemia, GI upset (abdominal pain, diarrhea, flatulence, n/v). [Taking each dose w/ plenty of fluid should decrease GI upset.]
Citalopram
Most Common: GI upset, xerostomia, either drowsiness or insomnia, profuse sweating. Sexual dysfunction can also occur (as impotence or ejaculation disorder), but it is reversible upon discontinuation.
Meloxicam
Most Common: GI upset (N/V/D/C, abdominal pain, heart burn), edema, headache or dizziness
Latanoprost
Most Common: blurred vision, burning/stinging, itching, increased pigmentation of the iris
Carvedilol
Most Common: hypotension, dizziness and fatigue, hyperglycemia, diarrhea, weight gain, muscle weakness
Oxycodone
Most Common: drowsiness, dizziness, nausea, vomiting, constipation. [Most side effects decrease over time, except constipation. Therefore most patients will require a stool softner and/or laxative w/ longterm opioid therapy.]

Norgestimate & Ethinyl Estradiol
Most Common: GI upset (N, V, abdominal cramps, bloating), breakthrough bleeding or altered menstrual flow (this will usually subside within 3 months for most patients), breast tenderness, headache, weight gain (which is cyclic, usually gain weight around menses), hyperkalemia (rare)

Most Common: GI upset (N/V/D), oral candidiasis (thrush)
Codeine & Guaifenesin
Most Common: drowsiness, dizziness, headache, GI upset (nausea, stomach pain, constipation)
Glyburide
Most Common: weight gain, hypoglycemia

Most Common: headache, nausea, sinusitis, vaginal secretions, vaginitis, weight gain
Bupropion
Most Common: rapid heart beat, headache, dizziness, insomnia, xerostomia, weight loss, nausea
Doxycycline
Most Common: photosensitivity, GI upset (dyspepsia, diarrhea, nausea), arthralgia, menstrual cramps, esophogeal irritation (take w/ a full glass of water and do not lay down directly after administration)
Enalapril
Most Common: GI upset (N/V/D, constipation), hypotension, chest pain, syncope (fainting), headache, dizziness, fatigue, rash, abnormal taste
