Adenosine

Class
Endogenous Nucleoside
Adenosine

Action
*Slows electrical impulses through the AV node and inhibits re-entry pathway converting SVT to NSR
*Onset= 30 sec
*Duration= 1-2 minutes
Adenosin

Indications
SVT
Adenosine

Contraindications
* 2nd and 3rd degree av heart blocks

* Sick Sinus Syndrome (without implanted pacemaker)
Adenosine

Side Effects
*SOB/Dyspnea; may cause bronchospasms in pt's with asthma (BHPO)

*Chest Pressure/Palpitations; May be acute in some pt's for brief period

*Dizzy/lightheaded/headache

*Nausea

*Transient arrythmias; (bradycardias, AV Blocks, Sinus Pause, Ventricular Ectopy) Generally not trated and are self limititng
Aspirin

Class
Salicilate
Aspirin

Action
* Inhibits the normal tendency for platelets to accumulate inside injured or occluded cardiac arteries thereby promoting better blood flow through the vessels to better profuse the heart

*onset= 15 to 30 minutes
*duration= several days antiplatelet effect
Aspirin

Contraindication
NONE
Aspirin

Side Effects
*GI Upset - indigestion, N/V, heartburn, Epigastric Pn

*Occult Bleeding
Atropine

Class
Anticholinergic
Atropine

Action
*Competes with acetylcholine for receptor sites blocking the PNS response at the SA & AV nodes
*increses heart rate by increaseing electrical conduction through heart
*Inhibits secretions by decreasing PNS effect on Bronchial, salivary, sweat and GI glands

*onset= 1 to 2 minutes IV
*Duration= 2-6 hours
Atropine

Indications
*Asystole - Pea wiht bradycardia <60

*Unstable Bradycardia

*OPP poisoning
Atropine

Contraindications
NONE
Atropine

Side Effects
*Tachycardia/palpations

*Dry mouth/ N/V /

*Pupil Dilation/ Blurred Vision

*Flushed/ Hot/ Dry skins
Dopamine
intropin

Class
Sympathomimetic

Both alpha and beta properties
Dopamine
intropin

Action
*in low doses, selectively dilates blood vessels supplying brain, kidneys, heart and GI tract
*in med to high doses, increases cardiac output by increasing contractility and stroke volume thereby increasing blood pressure
*at high doses causes vasoconstriction ad increased heart rate
*Onset= 5min
*duration= Effects stop approx 10 minutes after drip is stopped
Dopamine
intropin

Indications
*discomfort/pain of suaspected cardiac origin with associated shock

*Shock- normovolemic (neurogenic, anaphilactic, septic)

*bradycardia after max atropine
Dopamine
intropin

Contraindications
NONE
Dopamine
intropin

Side Effects
*at high infusion rates, hypertension & vasoconstriction may occur

*at low rate hypotension may occur

*tachycardias/ palpitations

*increased myocarial O2 demand
EPI

Class
sympathomimetic (both alpha and beta effects
Epi

Action
*on the bronchi- bronchodilation (beta 2)
*on the heart-
^increased heart rate beta1 - chronotropic
^increased contractility - inotropic
^increased AV conduction- dromotropic
^increased automaticity- dromotropic
*Onset- IV/ET 1-2min, SC 5-10min
*duration 5-10min
EPI

Indications

RESPRITORY
*resp distres with brochospasms
*allergic reaction with severe resp distress / bronchospasm or exsposure to allergen with precious severe reaction and onset of any allergic symptoms.
*resp distress with stridor in pediatric pt's
Epi

Indications
*Respritory distress with bronchospasms

*allergic reaction / anaphylaxis with severe resp distree/ bronchospasm or exsposure to allergen with previous severe reaction and onset to any allergic symptoms

*anaphylaxis

*pulseless rhythms, cardiac arrest, unmonitored arrest

*resp. distress in pediatric with stridor
EPI

contraindications
NONE
EPI

Side Effects
*anxiety/restlessness
*Palpit
