List 6 Early Morning (before bkfst) Duties
1. Take pt. To the BR or provide a urinal, bedpan or BSC.
2. Wash pt’s hands and face.

1. Removes body dirt
2. Eliminates body odors & cleans and refreshes the pt
3. Gives you the opportunity to provide ROM

5. Gives you a chance to observe the pt for any unusual body changes (rashes, bruises or redness)
Complete Bed Bath
• Water temperature in basin is 110 degrees
• Water is changed 3 times during the bath
• The CNA does the entire bath for the pt.
• Privacy (closed door, curtain, bath blanket) is provided at all times
• During the bath, lotion, deodorant, and ROM is provided to all extremities
CBB Continued
• Offer elimination before you begin the bath.
• Gloves are used when there are open, rash or dirty areas noted. They are also used if there is a chance of contact with a body fluid ie: peri/anal care
• Always raise the bed horizontally to a level that is comfortable for you
• Do not oversoap the water, causes dryness. Soap needed for pericare however use clear fresh water to rinse
Pericare
•Always use soap on a clean wash cloth and rinse with clear fresh water.
•Always wear gloves
•For female, always wash from front to back
•Female wash inside of labia then outside labia and then the groin. Turn on side & do anal care from front of body to the back
•Use the drape procedure to provide additional privacy
Pericare Continued
•Use same wash cloth/soap/rinse procedure as with a female pt.
•Push back the foreskin if pt is not circumcised and clean the tip of the penis in a circular motion then rinse and return the foreskin to natural position
•Then wash the penis shaft and then the scrotum, using a clean part of the wash cloth with each wipe. Rinse the area with clear fresh water.
•Wash the groin area from front to back and rinse
•Turn him on his side and wash the anal area from front of the body to the back
•Always remember that this is a gloved procedure and drape the pt for privacy just as you would a female
Assembling equipment
•The first thing you do ater you assemble your equipment is to wash your hands
•Make sure you have all equip and supplies available before you start any procedure
•Use gloves to wash out emesis or bath basins or those items that have had contact with body fluids
•Store items & equipment in bedside table
Tub Baths
•Water temperature in 105 degrees
•Pt is draped as in a shower procedure
•Two people need to attend bath at all times
•Disinfect chair and tub both before & after use
•Provide privacy with door & curtain
•Let pt wash what they can & you assist
Showers
•Drape pt in shower chair as pt is nude
•Push shower chair facing front
•Keep gloves in your pocket for use
•The procedure is as follows: Face, NECK, CHEST, ABD, ARMS/AXILLA, BACK (RINSE) LEGS JUST TO ANKLES, (RINSE), GLOVES FOR PERI/ANAL CARE, (RINSE) THEN FEET AND SHAMPOO.
Oral Hygiene
•Assemble your equipment
•Wear gloves at all times
•Dilute mouthwash with water
•Have pt rinse mouth
•Provide toothbrush for pt to brush own teeth
•Floss teeth that are natural, ad lib
•Rinse again with mouthwash
•Always provide a towel across pt’s chest
•Always provide a towel across pt’s chest
Dentures
•Assemble the equipment
•Wear gloves at all times
•Line both sink/emesis basin to prevent breakage of expensive dentures/partials warping of dentures
•Place dentures in denture cup & use emesis basin to provide oral hygiene before reinserting dentures
•Clean equipment & store in bedside table
Shaving a Man
•Assemble your equipment
•Obtain a new & disposable razor
•Cover chest with towel
•Fill bath basin with 110 degree water
•Apply gloves
•Use wash cloth with water and soften beard
•Apply shave cream, shave, rinse. Remove gloves
•Dry with towel and dispose razor in a sharps container
Bed Shampoo
•Assemble equipment & wash hands
•Provide privacy & place pt on side in demi fowlers position to prevent aspiration of fluids into the lungs. Place towel on chest. (Apply Gloves)
•Using a toothette, glycerin swab, or padded tongue blade, clean pt’s mouth
•Hold emesis basin under chin to obtain drainage during procedure
•Dry pt’s mouth with towel and keep gloves on and clean equipment. Return to bedside stand
Hair Care
Assemble equipment and wash hands gloves needed if sore/open areas on head place a towel across shoulders of pt. use appropriate comb & or brush section out hair and start at the bottom and comb out tangles first, one section at a time style hair as pt desires (age appropriate) store supplies in bedside stand
Finger nail Care
•Assemble equip, wash hands, wear gloves prn
•Place towel on surface, & remove any old nailpolish on nails. Place pt’s hands in warm soapy water by using an emesis basin. Use an orange stick to clean under nails and for cuticles prn. File nails for smoothness. Lotion hand and forearms and give massage. Apply nail polish that pt prefers. Cutting nails is not permitted as a student
Foot Care
•Assemble equip & wash hands
•Place chux on floor and place basin with warm water on floor & test temp
•Place un-socked foot in baisin and apply gloves
•Wash entire foot & toes with soapy wash cloth and rinse foot
•Dry entire foot with toes and apply lotion
•Dry with towel to remove excess lotion and clean and stroe equip in bedside stand
Dressing The Pt with Hemiplegia
• Assemble equip., wash hands, give privacy
•Undress the unaffected side first and then the affected side.
•When dressing, dress the affected side first and then the unaffected side
•Apply bra, shirt, and then pants, socks and shoes
