inhalation
If there is obstruction in intrathoracic airways (small airways) , when would you see it more, during inhalation or exhalation?
exhalation
What are some causes of extrathoracic airway obstruction?
- vocal cord paralysis
- goiter
If you see obstruction on both exhalation and inhalation, what type of obstruction is it?
Fixed obstrution
2 reasons for DLCO to be low:
1. architectural destruction (emphysema, pulmonary fibrosis, pulmonary vascular disease having to do with pulmonary hypertension)
2. extrinsic constriction ( can't take deep breath in due to obesity or scoliosis, chest wall abnormality for ex.)
How can you use DLCO to distinguish between COPD and asthma?
COPD - DLCO will be low
Asthma - DLCO will be normal or increased (breathing harder, more negative pressure intrathoracically, which increasing blood flow to right heart, which increases blood flow to lungs, which increases the amount of RBCs available to take up CO)
