Describe beach chair position
legs should be elevated as close as possible to the level of the heart and slightly flexed to prevent sciatic nerve traction
45-60 degrees, flexion at the hips and slightly flexed |
beach chair arm position and why
arms should be elevated to avoid traction of hte shoulder muscles and brachial plexus
what is beach chair position used for?
neurosurgical exposure of cspine, breast, sholder, arm, crani, thyroid
physiological considerations with beach chair position?
CV, neuro, resp, peripheral nerves |
CV- hypotension r/t venous pooling, decrease CO 10%, decrease preload
Neuro- pnumo resp- improves ventilation and access to airway, less facial swelling. peripheral nerves- brachial plexus compression at humoral joint, sciatic compression, ulnar nerve. |
indications for lateral position
thorax, kidneys, rectal, spine/crani, hips, shoulder
complications with lateral decubitus
neck angulation (occlusion of veterbral or arotid arteries which can increase ICP)
dependent lung is underventilated nondependent lung is over inflated v/q mismatch decrease SVR and BP eye pressure |
