Hemicholinium, Triethylcholine

Inhibitors of ACh synthesis. Pre-junctional acting agent (Inhibit transmission).

Vesamicol

inhibitor of ACh storage. Pre-junctional acting agent (Inhibits transmission).

Botulinum toxin

(lethal dose: 1 μg/kg) produced by Clostridium botulinum. Inhibits the binding of Ach vesicles with the cell membrane. Pre-junctional acting agent (Inhibits transmission). Acts for several months.Given into the muscle in cosmetic surgery, blepharospasm, strabism, spasticity. Also used in hyperhidrosis and overactive bladder. (Aminoglycosides and Mg++ions have the same effect)

D-tubocurarine

active molecule of curare toxin. Non-depolarizing blocking agent (muscle relaxant) – long duration of action. So it’s a reversible NM-ACh receptor antagonist. Benzyl isochinoline compound. Eliminated by kidneys and liver. It’s side effects make it unfit for clinical use.

Doxacurium

long duration of action, non-depolarising NMJ blocker (Nm-Ach receptors). Eliminated by the kidneys. Benzyl isochinoline compound.

Atracurium

Non-depolarizing blocking agent (Reversible Nm-Ach receptors antagonist) – Intermediate DoA. Benzyl isochinoline compound. Causes unwanted histamine release and siezures. Not used clinically.

Cisatracurium

Replaced atracurium clinically. Is a stereoisomer

Mivacurium

Non-depolarizing blocking agent (Reversible Nm-Ach receptors antagonist) – Short DoA. Benzyl isochinoline compound. Causes some unwanted histamine release. Not preferred clinically

Vecuronium, Rocuronium

Non-depolarizing blocking agent (Reversible Nm-Ach receptors antagonist). Ammonio-steroidal compound. Intermediate duration of action.

Sugammadex

Sugondese nuuuuts

binds steroidal relaxants (Vecuronium and Rocuronium) in the plasma, inhibiting the effect of non-depolarizing muscle relaxants by chemical antagonism. Used for decurarization.

The only depolarising blocking agent?

Suxamethonium (= succinylcholine)

Nicotinic R agonist. It keeps the inotropic receptor open, leading Phase I and Phase II blocks. Short DoA (5-10m).*Can cause malignant hyperthermia! (treated with: dantrolene, NaHCO3, physical cooling)

What else can block release of Ach from presynaptic neuron expect Botulinum Toxin?

Aminoglycosides and Mg2+

What is the side effect of the non-depolarising muscle relaxants?

Ganglion-blocking effects
