Used before a project is implemented. Ex) training, drafting instructions, etc.

Deliberate

Routine. Ex) QA, OJT, safety briefs.

Time Critical

Used during operational exercises. Ex) checklists.

Class A Mishap

Damage of $2 million or more.


Fatality or full disability.

Class B Mishap

Damage $500k < $2 million.


Permanent partial disability or 3 or more people hospitalized.

Class C Mishap

Damage $50k < $500k.


Non-fatal injury causing loss time of work. 5 or more days.
