What are the 3 factors affecting the active phase?
3P's: 1)Power 2)Passenger 3)Pelvis
What is the normal response to the NST (non stress test)? What is nonreactive, and what are the causes? x4
i)15 BPM increases x2 over 20 min period ii)NR means no accelerations and need BPP. Causes include i)GA<32 ii)sleeping iii)hypoxia iv)maternal sedation w/drugs
How many hours must pass in order to have a prolonged 1st latent stage of labor for primip and multip? What are 2 causes and what should you do? What is a cause for prolonged active 1st stage?
i)20 hours for primip; 14 hours for multip ii)a)early sedation w/anesthesia b)hypertonic uterine contractions. Give oxytocin iii)cephalopelvic disproportion
What is defn of 1st, 2nd, 3rd stages of labor?
i)1st stage: onset of labor to complete dilation (latent=onset to 3-4 cm and active is 3-4 cm to 10 cm) ii)2nd stage is fully dilated to delivery of infant iii)delivery of infant to delivery of placenta
What is a positive contraction stress test?
late decels after >50% of contractions (fetal jeopardy possible); must have at least 3 contractions in 10 min
What are the things tested in BPP? Test the Baby MAN!
fetal Tone
fetal Breathing fetal Movement Amniotic fluid volume Nonstress test |
What is the description of early decels and what is most common cause? Of variable decels? Of late decels?
i)decels occur at time of mom's contractions and ends at around same time. Due to cephalic compression ii)occur at any time during maternal contraction; due to umbilical cord compression (switch mom to side) iii)starts at peak of moms contraction and ends after mom's contraction (fetal hypoxemia and uteroplacental insufficiency. If repetitive and severe, deliver asap)
