Define primary amenorrhea

Failure to initiate menstruation by the age of 16 years in the presence of normal growth and development and normal secondary sexual characteristics or 14 years in the absence of other evidence of puberty.

Causes of primary amenorrhea

1. Constitutional: delayed puberty


2. Hypothalamus: stress, exercise, anorexia, bulimia (weight related), chronic illness, Kallman’s syndrome


3. Pituitary (ant): pituitary adenomas, hyperprolactinemia, hypothyroidism


4. Disorders of the ovaries: Gonadal agenesis/dysgenesis and mosaicism (Turner’s syndrome)


5. Disorders of the outflow tract: Mullerian agenesis/dysgenesis (May-Rokitanky-Kuster-Hauser Syndrome), transverse vaginal Septum, imperforate hymen (haematocolpos)
