Describe the compartments of the hand
- Intrinsic muscles of the hand are chiefly on the palmar aspect
- Thenar muscles in thenar compartments
- Hypothenar muscles in the hypothenar compartment
- Lumbricals in the central compartment
- Adductor pollicis in the adductor compartment
What muscles make up the thenar compartment and what is there innervation?
- Abductor pollicis brevis
- Flexor pollicis brevis
- Opponens pollicis
- Innervated by the Median nerve
Describe the muscles in the adductor compartment of the hand and their innervation
- Adductor pollicis
- Innervated by the ulnar nerve
Describe the muscles in the hypothenar compartment of the hand and their innervation
- Abductor digiti minimi
- Flexor digiti minimi
- Opponens digiti minimi
- Innervated by the ulnar nerve
Describe the muscles in the central compartment of the hand and their innervation
- Lumbricals = median (lateral 2) and ulnar (medial 2) nerves (T1)
- Interossei = ulnar nerve (T1)
Label this diagram
1.Abductor pollicis brevis
2. Flexor pollicis brevis 3.Opponens pollcis 4&5.Adductor pollicis
6. Abductor digiti minimi
7. Flexor digiti minimi
8. Opponens digiti minimi
Describe the features of the lumbricals
1. Origins
2. Insertion
3. Innervation
4. Action
- 4 Lumbricals on digits 2-5 (NOT thumb)
1. Origin = tendons of FDP
2. Insertion = extensor expansion
3. 1&2 supplied by median nerve C8 and T1, 3&4 supplied by deep branch of ulnar nerve C8 and T1
4. Actions = Flexion at the MCP joint and extension at the IP joint (opposite of this causes claw hand e.g. in ulnar nerve damage)
What are the palmar interossei?
- 3 muscles (NONE on thumb)
- PAD = adduct digits
- Innervated by ulnar nerve C8 and T1
What are the dorsal interossei?
- 4 muscles between metacarpals (1st is between 1st and 2nd metacarpal i.e. thumb and first digit)
- DAB = abduct digits
- Innervated by ulnar nerve C8 and T1
- Always check for 1st dorsal interossei wastage between thumb and forefinger as can show muscle wastage indicative of T1 stress of damage caused e.g. by superior lung tumours
Which intrinsic hand muscles are innervated by the ulnar nerve?
1. Lumbricals 3 & 4
2. Interossei (palmar and dorsal)
3. Adductor pollicis
4. Hypothenar muscles
Which intrinsic hand muscles are innervated by the median nerve?
1. Lumbrical 1 & 2
2. Thenar muscles (not adductor pollicis)
