Description/Mechanism of action


*Antidysrhythmic that suppresses automaticity and raises threshold of the ventricles.

Indications


*Pulseless ventricular tachycardia/ventricular fibrillation (alternative to Amiodarone)


*Ventricular tachycardia with a pulse (alternative to Amiodarone)


*Stable polymorphic VT if Torsade's do points is suspected

Contraindications


*Allergy to the medication


*Wide complex ventricular escape beats associated with bradycardia


*2nd or 3rd degree heart block


*Severe bradycardia

Precautions/Special notes/Side effects


Respiratory


*Use caution I patients with CHF


CNS


*Headaches


*Dizziness


MISC


*Renal impairment or Hepatic impairment (reduce "maintenance dose" only)


*May decrease ICP response during intubation


*Elderly or debilitated patients (1/2 dose for patients >75 years old)


*Family/patient history of malignant hyperthermia

*0.5-1mg/kg bolus. Repeat every 5 minutes as needed. Maximum of 3mg/kg

Adverse effects

*Respiratory depression/hypoxia


*Seizures (especially with overdose)


*Hypertension/Shock
