What is jaundice

Excessive amount of hemobilirubin or cholebilirubi in blood and tissue fluids causing yellow diascoloration

Types of jaundice

Hemolytic - prehepatic

Toxic - intrahepatic

Obstructive - posthepatic

Causes of hemolytic jaundice

Protozoa - piraplasmosis, babesiosis

Clostridium hemolyticum bovis - causes Bacillary hemoglobinuria

Hemolytic anemia - Rh nega mom makes antibodies againt Rh positive baby

Incomplete blood typing

Causes of toxic j

Salmonella

Hepatitis

Injured hepatic cells

Plant toxins (seneco, crotalaria)

Organic compounds (clay pigeons)

Causes obst. Jaundice

Parasitez - Ascaris, Thysanosoma actinoides, Fasciola

Tumors

Cholangitis and cholecystis

Normal RBC breakdown

Dead rbc > SPLEEN

Heme (5%) , Iron, Globin (95%)

Heme > porphyrin > Biliverdin > LIVER

Biliverdin + liver albumin > hemobilirubin > hemobilirubin + glucoronic acid > cholebilirubin > passed through bile > INTESTINES

Cholebilirubin > Urobilinogen > urobilin (poo) and stercobilin (pee)

Pathogenesis of hemolytic j

Too much RBC death that the liver cannot absorb and metabolize it

Pathog. Of toxic j

Hepotocyte necrosis which causes the cholebilirubin to escape through the hepatic cords and enters the sinusoids to adjacent organs

Pathog. Of obstructive j

Cholebilirubin cant enter the intestines bc of biliary obstruction

Dx for jaundice

Icteric index - plama color ( standard soln of potassium dichromate)

Van Den Bergh rxn - Erlich's reagent w plasma or serum to make color reactiom

What do u call the colored compound in Van Den Bergh rxn

Azorubin or azobilirubin

Interp of Van Den Berghs rxn

Direct - 1-2 mins : Pink -purple (cholebilirubin) obstructive

Indirect - 2 mins : no color, 10 mins : golden, 1hr : brown > pink

(Hemobilirubin) hemolytic

Biphasic - 2 mins : brown red

(Chole and hemobilirubin) toxic

Describe the colors of hematoidin and hemosiderin

Hematoidin - yellow and green

Hemosiderin - brown (Fe)

Microscopic app of jaundice

Heart failure cells

Blue granules

Hemosiderin granules
