Cyclosporine

Calcineurin Inhibitor -> binds CYCLOPHILIN and blocks T Cell activation by preventing IL-2 transcription.




Use: Transplant rejection prophylaxis, psoriasis, RA.




Toxicity: NEPHROTOXICITY, HTN, hyperlipidemia, Hyperglycemia, Tremor, Hirsuitism, Gingival Hyperplasia.

Tacrolimus

Calcineurin Inhibitor -> binds FK506 binding protein (FKBP) -> blocks T cell activation by preventing IL-2 Transcription.




Use: transplant rejection prophylaxis.




Toxicity: Nephrotoxicity, increased risk of DIABETES and NEUROTOXICITY.

Sirolimus (Rapamycin)

mTOR Inhibitor -> binds FKBP -> blocks T Cell activation and B Cell differentiation by preventing IL-2 Signal Transduction.




Use: KIDNEY TRANSPLANT rejection prophylaxis




Toxicity: ANEMIA, thrombocytopenia, leukopenia, Insulin resistance, hyperlipidemia.




NOT nephrotoxic.

Basiliximab

Monoclonal antibody that blocks IL-2 Receptor.




Use: Kidney transplant rejection prophylaxis.




Toxicity: edema, HTN, tremor.

Azathioprine

Antimetabolite precursor of 6-MP -> inhibits lymphocyte PROLIFERATIOn by blocking nucleotide synthesis.




Use: transplant rejection prophylaxis, RA, Chron's Disease, Glomerulonephritis, autoimmune disorders.




Toxicity: Leukopenia, anemia, thrombocytopenia.




METABOLIZED BY XANTHINE OXIDASE -> increased toxicity with Allopurinol use.

Glucocorticoids

Inhibit NF-kB -> suppress BOTH B and T cell function by decreasing transcription of many cytokines.




Use: Transplant rejection prophylaxis, autoimmune disorders, inflammation.




Toxicity: HYPERGLYCEMIA, osteoporosis, central obesity, muscle breakdown, PSYCHOSIS, acne, HTN, CATARACTS, peptic ulcers.




Can cause iatrogenic CUSHING Syndrome

Use: Anemias (esp in RENAL FAILURE)
