H2 blockers (end in "tidine")
cimetidine
ranitidine
famtidine
nizatidine
- reversible block of H2 receptors leads to decreased H+ production by parietal cells
-used for peptic ulcer, gastritis, esophageal reflux
-tox: cimetidine inhibits P450, and also has antiandrogenic effects (prolactin release, gynecomastia), it can also cross BBB to cause confusion, headaches, dizziness. can also cross placenta
-both cimetidine and ranitidine decreased renal excretion of creatinine
proton pump inhibitors
omeprazole
lansoprazole
- irreversibly inhibit H/K ATPase in parietal cells
-used for peptic ulcer, gastritis, reflux, zollinger ellision syndrome
bismuth, sucralfate
- bind to ulcer base providing physical protection and allows HCO3 secretion to reestablish pH gradient in mucous layer
- used for ulcer healing, traveler's diarrhea
triple therapy for H.pylori ulcers
amoxicillin
metronidazole (or tetracycline)
bismuth

also use PPI
misoprostol
- a prostaglandin (PGE) analog
-increases secretion of mucous barrier and decreases acid production
- used for prevention of NSAID induced peptic ulcer, maintenance of patent ductus arteriosus
tox: diarrhea
muscurinic antagonists
pirenzepine
propantheline
atropine
- block M1 receptors on ECL cells thereby decreasing histamine production
-block M3 receptors on parietal cells therby decreasing H production
-used for peptic ulcer
tox: tachycardia, dry mouth, difficulty focusing eyes
anatcids
aluminum OH
magnesium OH
calcium carbonate
- AlOH- constipation, hypophosphatemia, proximal muscle weakness, seizures
- MgOH- diarrhea, hyporeflexia, hypotension, cardiac arrest
- CaCO3- hypercalcemia, rebound acid increase
infliximab
- monoclonal Ab to TNF. anti inflammatory
-used for Chron's disease, RA
- tox: resp infection, fever, hypotension, reactivation of latent TB
sulfasalazine
- a combo of sulfapyridine and mesalamine. activated by colonic bacteria
- used for ulcerative collitis, chrons
- tox: malaise, nausea, oligospermia, bone marrow suppression
ondansetron
- serotonin antagonist. powerful central acting anti-emetic
- control vomiting postop and in pts undergoing chemo
- tox: headache, constipation
metoclopramide
- D2 receptor antagonist. increase resting tone, contractility, LES tone, motility
- used for diabetic & post surgery gastroparesis
- tox: increase parkinsonian like effects, restlessness, nausea, diarrhea. DRUG INTERACTION WITH DIGOXIN & DIABETIC AGENTS. contraindicated in patients with small bowel obstruction
