Under what conditions can the board refuse to grant or renew a license?


  • use of fraud or deception in the application process
  • conviction of a felony
  • practice under a false or assumed name
  • impersonating another provider
  • habitual use of alcohol or drugs that effects performance of duties
  • neglect or wrongful action in performing duties

licensee is incapacitated due to substance use or mental disorder?


  1. Require they submit to an eval.
  2. Eval results are not privileged and can/will be admissable in a hearing by the board
  3. Failure to submit to the exam will result in summarily suspension of license
